Hemodializin farklı hasta gruplarında LP(a)' yı yükseltici etkisi

Özet
AMAÇ: Düşük dansiteli lipoprotein (LDL) 'ye benzer özellikleri ile aterojenik, plazminojene benzer özellikleri ile trombojenik olan Lipoprotein(a) nın [Lp(a)]'nın, hemodializ sırasında yükseldiği bazı çalışmalarda gösterilmiştir. Bu çalışma hemodializin Lp(a) üzerindeki akut etkisini, riskli hasta gruplarında değerlendirmek amacıyla yapılmıştır. GEREÇ VE YÖNTEM: Hemodialize ilk kez giren, 70 kişilik kronik böbrek yetmezlikli hasta, diabetes mellitus+vasküler hastalığı olan (n = 12), sadece vasküler hastalığı olan (n=8), diabeti olan ve olmayan vasküler hastalığı (+) olan hastalar (n=20), diabeti ve vasküler hastalığı olmayan (n=50 )hastalar olarak gruplandırılmıştır. Bu hasta gruplarında dializ öncesi ve dializ sonrası Lp(a) değerlerine bakılmıştır. BULGULAR: Tüm hasta gruplarında dializ sonrasında Lp(a) 'da anlamlı yükselme olmuştur (sırasıyla p 0.05). SONUÇ: Hemodializin Lp(a) 'yi akut yükseltici etkisi vardır. Bu yükselmenin nedeni halen kesin olarak bilinmemekle birlikte, diabet ve damar hastalığı gibi ek hastalıkların varlığı ve bunların sayısının artması ile bu olumsuz etki belirginleşmektedir. Bu akut olumsuz etki, uzun dönemde diabeti ve damar hastalığı olan riskli hasta grubunda kardiovasküler mortalite ve morbiditeyi daha fazla arttırabilir.
Abstract
OBJECTIVE: Lipoprotein (a) [Lp(a)] is atherogenic as little dansity lipoprotein (LDL) like particle and thrombogenic as plasminogen. In some studies, it is shown that Lp(a) is elevated during the hemodialysis procedure. This study is performed to see the acute effect of hemodialysis on different risky patient groups and to determine the difference between them, if there is. MATERIAL AND METHODS: 70 chronic renal failure patients, who would be dialysed for the first time, are chosen. Twelve of them had both diabetes mellitus and vascular disease, 8 of them had only vascular disease, 20 of them had vascular disease with diabetes or not, 50 of them had neither diabetes nor vascular disease. Pre and post dialysis values were compared, for all groups. RESULTS: There was a significant increase for all patients after dialysis (p<0.0l,<0.05,<0.05,<0.05 respectively). But the difference of elevation level was quantitatively higher than the other groups, in the group of patients with diabetes and vascular disease (p<0.05). Vascular disease or diabetes had no significant different effect on predialysis levels of Lp(a) (p>0.05). CONCLUSIONS: Hemodialysis has an acute negative effect on the patients. The cause of this acute increase is still unknown. This acute harmful effect became more significant in the presence of additional disorders like diabetes or vascular disease or both. In the long term, this acute negative effect may increase the mortality and morbidity of these diabetic and vascular disease positive risky patients.
