Özet
Endoskopi, halen Özofagus ve kardia kanserlerinin en önemli tanı aracı olma özelliğini korumaktadır Ancak, sağlıklı veriler olmadığı için, ülkemiz genelindeki epidemiyolojik, histopatolojik, endoskopik ve klinik özellikler hakkında gerçekçi yorumlar yapmak zordur. AMAÇ VE YÖNTEM: Bu konudaki eksikliği bir ölçüde gidermek, endoskopik-patolojik tanı uyumunu saptamak ve koordineli olarak gerçekleştirilecek benzer çalışmaları teşvik etmek amacıyla planlanan bu çalışmada, bilim dalımız İbn-i Sina Hastanesi endoskopi laboratuarında Ocak 1993-Aralık 1997 tarihleri arasında özofagogastroduodenoskopi uygulanarak, Özofagus veya Kardia Ca tanısı konan ve biyopsi ve/veya sitoloji yapılan 112 hastadan histopatolojik değerlendirilmeleri fakültemiz Patoloji ABD'nca gerçekleştirilen 80'i retrospektif olarak değerlendirmeye alındı. BULGULAR: Adı geçen 80 hastanın 53'ünde (% 66.25) endoskopik tanı doğrulanırken, 8'inde (% 10) tanı şüphesi nedeniyle (orta-ağır displazi) biyopsi tekrarı önerilmiş, 19'unda (% 23.75) ise endoskopik tanı doğrulanamamıştır. 1/3 alt özofagus ve kardia ağırlıklı yerleşim gösteren tümörlerde en fazla (% 37.8) az diferansiye adeno Ca saptandı. SONUÇ: Çok merkezli çalışmaların desteklenmesi ve koordinasyonu, ülkemizde pek de nadir olmayan ve yaklaşık %76 oranında endoskopik kesin tanı koyabildiğimiz Özofagus ve kardia Ca'ların epidemiyolojik, klinik ve histopatolojik özelliklerinin daha sağlıklı dokümante edilmesine büyük katkılar sağlayacaktır.
Abstract
Endoscopy is still the most important tool in the diagnosis esophageal and cardiac cancers. However, because lack of qualified documentation, it is quite difficult to comment on the epidemiologic, clinical and histopathologic features of these two types of cancers in our country. AIM: The aim of this study was to in some way the gap of information as well as to stimulate similar, multicentric and coordinated trials concerning epidemiologic, clinical and histopathologic properties of the disease and also to evaluate the similarity between endoscopic and pathologic diagnosis. MATERIAL AND METHOD: 112 patients who had undergone upper gastrointestinal (GI) endoscopy and diagnosed as esophagus and cardia Ca and biopsied (with or without cytology) in the laboratories of İbn-i Sina Hospital of Ankara University between January 1993 and December 1997 participated in the study. Eighty patients whose biopsy/cytology specimens were examined in Pathology Unit of the same hospital were evaluated. RESULTS: Endoscopic diagnosis were confirmed in 53 (66.25 %) cases, re-biopsies were proposed in 8 (10%) patients because of moderate-severe dysplasia and endoscopic diagnosis were not confirmed in the remaining (23. 75 %). Poorly-differentiated adeno Ca was the most common type (37.8%) and tumors were frequently localized in the lower third of the esophagus as well as in cardia. CONCLUSION: Esophagus and cardia cancers are not uncommon and they can be diagnosed at a rate of 76% by upper GI endoscopy. For more accurate information and proper documentation, there is need for the stimulation of multicentric, coordinated trials throughout the country.
