Çocuklarda hızlı inhalasyon indüksiyonu ve intravenöz indüksiyon; %8 sevofluran, %5 halothan ve propofol karşılaştırılması

Özet

AMAÇ: Halotan çocuklarda hem anestezi indüksiyonunda hem de anestezi idamesinde kullanılan referans bir ilaç iken, sevofluran son dönemde klinik kullanıma giren, düşük kan çözünürlüğü sayesinde hızlı indüksiyon sağlayabilen bir inhalasyon anesteziğidir. Bu çalışmanın amacı çocuklarda halothan, sevofluran ve propofol'ün indüksiyon karakterlerinin belirlenmesidir. GEREÇ VE YÖNTEM: Çalışmaya elektif adenoid-tonsilektomi operasyonu geçiren 99 çocuk kabul edildi. Çocuklar kullanılan indüksiyon ajanına göre 3 gruba ayrıldılar. Buna göre Grup S'de (n=33) %8 sevofluran, Grup H'de (n=33) %5 Halothan ve Grup P'de (n=33) ise 5mg/kg propofol anestezi İndüksiyonu için kullanılmıştır. Hastalara premedikasyon ve kas gevşetici uygulanmadı İndüksiyon sırasında entübasyon sonrası 1, 3, 6. dakikalarda hemodinamik ölçümler yapıldı. Entübasyon sonrası entübasyon kondüsyonu, gözlenen komplikasyonlar ve boğuşma skorları incelendi. Kirpik refleksi kaybı, maske kabul ve entübasyo süreleri incelendi. Grup H'de indüksiyon esnasında hem bazal ölçümler ile hem de Grup S ile karşılaştırıldığında belirgin bir kardiyovasküler depresyon gözlendi. Anestezi indüksiyon süreleri inhalasyon anesteziklerinin kullanıldığı gruplarda benzer ancak propofol kullanılan grupta belirgin olarak kısa bulunmuştur (GrupS=359±69 sn, GrupP=103±45 sn, BULGULAR: Grup H=354±38 sn) (p<0,01). Entübasyon koşuları yine halotan ve sevofluran için benzer iken propofol grubunda daha kötü bulunmuştur (Grup S=4,27±1, 79, Grup P=5,36±1,47, Grup H=4,30±1,03) (p<0,01). SONUÇ: Sevofluran yumuşak anestezi İndüksiyonu, benzer indüksiyon süresi, stabil hemodinamik profili ve yan etkilerinin olmaması nedeni ile halotana alternatif olarak gözükmektedir.

Abstract

AIM: Halothane is a popular inhalation anesthetic in pediatric patients for induction and maintence of anesthesia. Recently sevoflurane has been referred as an rapid induction agent because of it low blood solubility. The aim of this study was to compare the induction charecteristics of halothane sevoflurane and propofol. MATERIAL AND METHOD: Ninety-nine children schuled for elective adenotonsillectomy were enrolled in the study. Patients were randomized into three groups due to agents used for anesthesia induction; group S (n=33) 8% sevoflurane, group H (n=33) 5% halothane, and group P (n-33) 5 mg/kg propofol. Premedication was not used. During the intubation and after on 1, 3, and 6th minutes hemodynamic parameters were noted. After intubation, the intubation condition score, the struggling score, and the complications score were also noted. The time to loss of eyelash reflex, face mask acceptance and intubation were also recorded. RESULTS: A significant cardiovascular depression was observed in group H during induction compared with group S and basal measurements. The anesthesia induction time was smilar in inhalation anesthetic groups and it was significantly shorter in group (Group S=359$\pm$69 sec, Group P=103$\pm$45 sn, Group H=354$\pm$38 sn) (p<0,01). The intubation conditions were also smilar in group H and S an significantly better the group P (Group S=4,27$\pm$1,79 Group P=5,36$\pm$1,47 Group H=4,30$\pm$1,03) (p<0,01). CONCLUSIONS: Sevoflurane is alternative agent to halothane for anesthesia induction in children, with smooth anesthesia induction, smilar induction times, stable hemodynemic profile and less complications.
