Özet
Katastrofik antifosfolipid sendromu tekrarlayan düşükler, arteryel ve venöz trombozlar, tormbositopeniye ilaveten geniş vasküler tıkanıklıklar, akselere veya malign hipertansiyon, renal yetersizlik, deri ülserleri ve gangrenlerle birlikte olabilen bir klinik tablodur. Primer veya sekonder olabilir. Sekonder tablolar sıklıkla bağ dokusu hastalıkları [özellikle sistemik lupus eritematoz (SLE), lupus like sendromu], Behçet hastalığı, bazı infeksiyonlar, bazı ilaçların kullanımında görülebilir. İster primer ister sekonder antifosfolipid sendromu (AFS) olsun tedavide mutlaka antikoagülan ve antiagregan verilmelidir. Eğer tablo sekonder AFS ise tedaviye kortikosteroid ilave edilebilir. Biz bu yazımızda kliniğimize sistemik lupus eritematoz seyrinde ortaya çıkan, akselere hipertansiyon, tekrarlayan iskemik serebrovasküler inme, parmak amputasyonlarının eşlik ettiği katastrofik antifosfolipid sendromu olgusunu tartıştık. Hastamız steroid, warfarin ve aspirin ile başarılı bir şekilde tedavi edildi.
Abstract
Catstrophic antiphospholipid antibodies syndrome is recognised with recurrent fetal loss, arterial and venous thrombosis, thrombocytopeni with a large vascular occlusions, accelerated or malignant hypertension, renal failure, skin ulcerations and gangrens. It can be primary and secondary. Secondary forms can be observed in patients with collagen vascular disease (especially SLE, lupus like syndrome), Behçet's disease, some infections, and during usage of some drugs. Aspirin and wafarin should be used in all forms of AFS, it doesn't matter if it is primary or secondary. In the case of secondary AFS, steroid can be added to therapy. We report about a case with catastrophic antiphospholipid syndrome who developed accelarated hypertension, recurrent ischemic cerebrovascular stroke, and digital gangrens in the course of systemic lupus erythematosus. Our patient was treated succesfully with steroids, warfarin and aspirin.
