Özet

Ekinokokkus dünyada sık görülen ciddi bir paraziter enfeksiyondur. İki tip ekinokokkus türü klinik bulgu meydana getirir: Ekinokokkus granülosus (uniloküler veya kist hidatik) ve daha malign olarak adlandırılan ekinokokkus multilokülaris. Alveolar hepatik hastalıkta kist prolifere olarak uzak metastaza yol açabilmektedir. Prolifere olan kist kitle görünümüne yol açarak malignite ile karışabilmektedir. İki aydır devam eden karın ağrısı, kilo kaybı, ateş yüksekliği şikayetleri ile kliniğimize başvuran 8 yaşındaki kız olgunun yapılan fizik muayenesinde hepatosplenomegali saptandı. Olgunun batın ultrasonografisinde; karaciğer (KC) sağ lobda multipl solid nodüler yapılar görülerek KC tümörü düşünüldü. Batın bilgisayarlı tomografi ve MRI' da; KC sağ lobda multipl, opak madde tutmayan diffüz heterojen lezyon saptandı. Alfa feto protein ve human koryonik gonodotropik hormon normal bulundu. Tümör markerlerinin negatif oluşu ve görüntüleme yöntemlerinde kontrast madde tutmayan bir lezyon olması nedeni ile alınan kist hidatik serolojisine yönelik testlerle alveolar kist hidatik tanısı konuldu. Albendazol tedavisi başlanan olguda cerrahi tedavi programlandı. Ekinokokkus multilokülaris (alveoler kist hidatik) çevre dokulara invazyon göstermesi, uzak metastaz yaparak fatal seyir gösterebilmesi ve sıklıkla malignite ile karıştırılması nedeni ile ilginç bulunarak sunuldu

Abstract

Echinococcosis (hydatid disease or hydatidosis) is the most widespread, serious human cestode infection in the world. It is a zoonosis trasmitted from domestic and wild canid animals. Two Echinococcus species are responsible for distinct clinical presentations; E.granulosus (unilocular or cystic hyatid disease) and the more malignant E. Multilocularis (alveolar hydatid disease). Reproduction in E.Multilocularis resembles malignancy. The secondary reproductive units bud externally and are not confined within a single well-defined structure. Further, the cyst tissues are poorly demarcated from those of the host. This makes these cysts unsuitable for surgical removal. The secondary cysts are also capable of distant metastatic spread. The growing cyst mass eventually replaces a significant portion of the liver and compromises adjacent tissues and structures. A 8 year old girl suffered from abdominal pain, loss of weight and fever for two months. Physical examination revealed hepatosplenomegaly. Abdominal usg showed multiple solid nodular structures in hepatic right lobe. The patient was considered as having hepatic malignancy. CT scans and MR imaging findings were similar to USG findings and were consisted of multiple diffuse heterogen lesions in right hepatic lobe. ÁFP and âHCG were normal. Because of the negative markers serologic studies were done and alveolar cyst hidatid disease was diagnosed. Albendazole therapy was initiated. With the medical therapy progression has slowed down and removal of the infected tissue was planned. Because of specific features of the alveolar hydatiol disease, such as distant metastatic spread of the secondary cysts resembling malignancy, our patient is found interesting.
