Fibromyaljinin irritabl barsak sendromunda yaşam kalitesine etkisi

Özet
AMAÇ: İrritabl barsak hastalığı (İBS) olanlarda fibromiyalji sıklığının arttığı bilinmektedir. Bu çalışmada fibromyaljinin İBS hastalarında yaşam kalitesi üzerine etkileri araştırıldı. GEREÇ VE YÖNTEM: S.B. Ankara Eğitim ve Araştırma Hastanesi 1. Dahiliye polikliniğine karın ağrısı veya rahatsızlık ve yaygın vücut ağrısı şikayetleriyle başvuran 18 hasta çalışmaya alındı. İBS ve fibromyalji tanısı alan hastalar 1. grup, fibromyalji olmayan İBS'li hastalar ise 2. grubu oluşturdu. Her iki grupta dokuzar hasta (yedi kadın, iki erkek) mevcuttu. Grup 1 ve 2 de yaş ortalamaları sırasıyla 39.5±10.5 ve 39.5±9.7 yıl (p>0.05) idi. Hastalara İBS tanısı Roma Kriterleri ile fibromyalji tanısı American Collage of Rheumatology (ACR) kriterlerine göre konuldu. Hassas nokta ölçümleri önce elle muayene ile ve daha sonra her bir hastaya Fischer'in geliştirdiği Doku Kompliyansmetresi ile yapıldı. Hastalara SF-36, SCL-90 yaşam niteliği skalaları, Beck Depresyon Ölçeği (BDÖ) ve Çok Yönlü Beden Self İlişki Ölçeği (ÇYBSİÖ) uygulandı. İstatistiki analizde t testi kullanıldı. BULGULAR: Her 2 grupta premenstrüel sendrom, dismenore, kronik baş ağrısı, sicca sendromu ve temporomandibuler ağrı sorgulandı. Grup I'de dismenore daha sık bulunurken (p<0.05), diğer yakınmalarda farklılık yoktu. Yaşam kalitesi parametrelerinde her iki grup arasında istatistiksel olarak anlamlı farklılık saptandı (p<0.05). SONUÇ: Tüm skala ve ölçeklerde İBS ve fibromyalji birlikteliğinin, fibromyalji olmayan İBS'li hastalara göre yaşam kalitesi üzerine olumsuz etkisi saptandı
Abstract
PURPOSE: It is known that the prevalence of fibromyalgia is increased in patients with irritable bowel syndrome. In this study, we investigated the effect of fibromyalgia on quality of life in patients with IBS. MATERIALS AND METHODS: Eighteen patients who attended to Ankara Research and Training Hospital First Internal Medicine Department with complaints of abdominal pain or discomfort and musculoskeletal symptoms were assessed. First group consisted of patients with both IBS and fibromyalgia and the second only IBS patients. There were nine patients (7 female, 2 male) in both groups. The mean age in group 1 and 2 were 39.5 + 10,5 and 39,5 + 9,7 (p<0.05) respectively. The IBS was diagnosed by Rome criteria and fibromyalgia by American College of Rheumotology. Tenderness of points was performed by thumb palpation by Fischer. The patients completed SF- 36, SCL-90 quality of life scores and Beck Depression Inventory and the Multidimensional Body Self Relantions Questionnaire (MBSRQ) scalas. Statistical analysis was performed by t test. RESULTS: The premenstrual syndrome, dysmenorrhea, chronic headache, sicca syndrome and temporamandibular pain were evaluated in all patients. While the prevalence of dysmenorrhea was higher in the group 1 (p<0.05) the others symptoms were similar in both groups. The quality of life scores were statistically different between the two groups (p<0.05). CONCLUSION: We found that concurrence of IBS and fibromyalgia had negative effects on all scales and quality of life scores.
