Özet
Amaç:Çalışmamızda ilk olarak, GDM' li gebelerde gözlenen bir takım demografik ve obstetrik risk faktörlerinin GDM' li olmayan gebelere göre hangi oranlarda mevcut olduğunu saptamayı amaçladık.İkinci olarak da, gebelikteki glukoz intoleransının fetomaternal ve neonatal morbidite üzerindeki etkilerinin incelenmesi ile bu etkileri hem normoglisemik gebelerden oluşan gebeler hem de diğer benzer çalışmaların sonuçları ile karşılaştırmayı hedefledik. Gereç ve Yöntem:Bu çalışma 184 GDM' li gebeyi ve kontrol grubu olarak da 164 diabetik olmayan gebeyi kapsadı.GDM' li gebeler ve glukoz tolerans testi (GTT)' ndeki açlık kan şekeri (AKŞ) değerlerine göre AKŞ' si <105 olanlar White klas A1 ve AKŞ' si $\geq$ 105 olanlar White klas A2 olacak şekilde iki gruba ayrıldı.White klas A1 GDM' li gebeler diet, White klas A2 GDM' li gebeler ise hem diet hem de insülin tedavisi aldı.Yaş, kilo, obstetrik, fetomaternal ve neonatal komplikasyonlar White klas A1, A2 ve kontrol grubundaki gebeler arasında karşılaştırıldı. Bulgular:Diabetik olmayan gebeler ile karşılaştırıldığında, White klas hem A1 hem de A2 GDM' li gebelerin yaş ve kilolarının daha yüksek olduğu saptandı.Preeklampsi, polihidramnios ve oligohidramnios gibi obstetrik komplikasyonlar her iki DGM grubunda da kontrol grubuna göre yüksek oranda tespit edildi.Fetal makrozomi ve operatif doğum oranları GDM' li gebelerde kontrol grubu gebelere göre istatistiksel olarak anlamlı miktarda yüksek bulundu. Tartışma:Glisemik kontrolleri farklı da olsa, hem A1 hem de A2 GDM' li grupta obstetrik ve neonatal komplikasyon oranlarının kontrol grubundan yüksek bulunması anormal GTT' nin fetomaternal ve neonatal morbidite riskini arttırabilecek bir takım ciddi risk faktörlerini taşıyan hastalar için bir belirteç olduğu görüşünü desteklemektedir.Bu nedenle ister White klas A1, ister White klas A2 olsun, GDM' li tüm gebeler dikkatli bir şekilde takip ve tedavi edilmelidir.
Abstract
Objective:The aim of this study is first to determine the presence of obstetric and demographic risk factors in patients with gestational diabetes mellitus (GDM) in comparison with healthy pregnant women.The second aim is to elucidate the effects of glucose intolerance on fetomaternal and neonatal outcome in GDM and also to compare them with the results of similar studies. Marerıals and Methods:This study included 184 antenatal patients with GDM matched to 164 non-diabetic controls.Patients with GDM were subgrouped into White class A1 group and White class A2 group according to the fasting blood glucose (FBG) level on the glucose tolerance test (GTT), whether < or $\geq$ 105 mg/dl, respectively.Patients in White class A1 group treated with diet and patients with White class A2 group were treated with diet and insulin together.Age, weight, obstetric risk factors, fetomaternal and neonatal copmlications were compared between White class A1, A2 and control groups. Results:Compared with non-diabetics, patients with both White class A1 and A2 GDM were older in age and have higher weight.Obstetric copmlications as preeclampsia, polihydramnios and oligohydramnios were observed more in both GDM groups than control group.Fetal macrosomia and operative delivery were also significantly higher in GDM groups than controls. Conclusıon:Although glycemic controls differ, both White class A1 and A2 GDM patients have higher incidence of obstetric and neonatal copmlications than non-diabetic controls.This supports the thought that an abnormal GTT may be a marker for patients who have other significant risk factors that, by itself, can increase the risk of fetomaternal morbidity.That' s why all patients with GDM, whether White class A1 or A2, must be managed carefully.
