Mastektomi sonrası dren çekilme süresinin seroma oluşumu ve yara enfeksiyonu üzerine etkisi

Özet
Amaç: Bu prospektif randomize çalışmada, Modifiye Radikal Mastektomi (MRM) yapılan hastalardan bir grupta dren çekilme süresi kısa tutularak bunun seroma, yara enfeksiyonu ve nekroz gelişimi üzerine etkilerinin araştırılması amaçlanmıştır. Gereç ve Yöntem: Çalışmaya invazif meme kanserli 74 kadın hasta dahil edildi. Tüm hastalara Stewart kesi ile MRM ameliyatı yapıldı ve sahaya kapalı sistem, vakumlu, ucunda iki ayrı silastik dren bulunan drenaj sistemi yerleştirildi. Hastalar iki gruba randomize edildiler: Birinci grupta (n=31) drenaj miktarına bakılmaksızın drenler postoperatif üçüncü günde, ve ikinci grupta (n=43) drenler günlük drenaj 50 ml'den az olduğunda çekildi. Hastalar kolleksiyon, enfeksiyon ve nekroz gelişimi açısından takip edildiler. Oluşan seromalar günlük ponksiyonlarla boşaltıldı. Toplam drenaj süresi drenlerin kaldığı gün ve ponksiyon yapılan günlerin toplamı olarak alındı. Bulgular: Birinci grupta ortalama 8.7±4.3 gün süreyle 110±24 ml ortalama drenaj ve ikinci grupta 9.2±4.1 gün süreyle 107±34 ml ortalama drenaj gözlendi. Enfeksiyon birinci grupta 2 (%6.8), ikinci grupta 4 (%9.42), hastada ve nekroz birinci grupta 2 (%6.8), ikinci grupta 3 (%7.14) hastada gelişti. İki grup arasında ortalama drenaj miktarı, toplam drenaj süresi, enfeksiyon ve nekroz gelişimi açısından istatistiksel anlamlı fark saptanmadı. Sonuç: Bu sonuçlar drenlerin yabancı cisim reaksiyonu ile seroma oluşumunu etkilemediğini düşündürmektedir. Bununla beraber mastektomi sonrası drenlerin günlük 30 ml den daha az getirdiğinde çekilmesi pansuman süresi ve hasta konforu açısından avantajlı görünmektedir.
Abstract
Objective: This prospective randomized study aims to invastigate the effects of early drain removal in a group of patients having modified radical mastectomy (MRM) operation, on seroma collection, wound infection and necrosis. Material and Method: 74 female patients with invasive breast cancer were included in the study. MRM by a Stewart incision were performed and silastic drains connected to a closed vacum system were used in all patients. Then the patients were randomized into two groups. In the first group (n=31) drains were removed on the 3rd postoperative day regardless of yield and in the second group (n=43) drains were removed when daily drainage is lesser than 50 ml. Patients were followed for collection, wound infection and necrosis. Collected seromas were removed by daily punctures. Total drainage time was calculated as sum of days drain left in place and days of puncture. Results: The mean total drainage time in the first group was 8.7±4.3 day and the mean amount was 110±24 ml, whereas 9.2±4.1 days and 107±34 ml in the second group. Wound infection was noticed in 2 (6.8%) patients in the first group and 4 (9.42%) in the second group. Necrosis was seen in 2 (6.8%) patients in the first group and 3 (7.14%) in the second group. No statistically significant difference was noticed between the two groups for drainage, drainage duration, infection and necrosis. Conclusion: These results showed that drains do not effect seroma collection as a foreign body. It is concluded that timing of drain removal when drainage drops to 30 ml daily seems to be advantageous in respect to patients' comfort and wound caring time.
