Özet

Amaç: Çalışmamızda evde doğan ve değişik nedenlerle servisimize yatırılan yenidoğanların sorunlarını incelemeyi amaçladık. Gereç ve Yöntem: Haziran 2000-Mayıs 2001 tarihleri arasında yatırılan 525 hastanın dosyaları retrospektif olarak incelendi ve doğumları evde gerçekleşen 66 yenidoğan çalışmaya alındı. Olguların özellikleri, tanı ve sonuçları, sosyoekonomik ve kültürel durumları irdelendi. Bulgular: Yatırılan yenidoğanların %12.5 evde doğmuştu, geliş tartıları 1050g- 4600 g arasında bulunurken, geliş yaş ortalamaları 8.6 gündü. Annelerin yaş ortalaması 25.9 yıl olup, %45.5'i okur yazar değildi. Babaların yaş ortalaması 29.8 yıl olup, %15'i okur yazar değildi. Babaların %9'u işsizdi ve aralarında memur yoktu. Ailelerin %73'ü Doğu ve Güney Doğu Anadolu'dan göç etmişti. Ev doğumlarının %19.7'si (13/66), hastane doğumlarının %6.5'i (30/459) kaybedildi (p=0.0001). Evde doğan bebekler sepsis (n=2), yenidoğan hemorajik hastalığı (n=2), pnömoni (n=1), prematürite ile asfiksi (n=5), yenidoğan tetanosu (n=1) ve anomali (n=2) nedeniyle kaybedildi. Sonuç: Annelerin eğitilmesi, gebelikte bakım, uygun şartlarda doğum ve doğum sonrası yenidoğan bakımı ile yenidoğan döneminde gelişecek yenidoğan ölümleri azaltılmış olacaktır.

Abstract

Objective: This study aims to find out extent of health problems of hospitalized newborns whose deliveries were at home by untrained persons and to contribute to literature about the main problems of these newborns. Materials and Methods: The files of all home-born infants (66 out of 525), who admitted to our Neonatology Department between June 2000 and May 2001 were evaluated retrospectively. Admitting ages, weights, gestational ages, complaints, and places of birth of the newborns were reviewed. Furthermore, education levels, occupations, and kinships of parents were recorded. Results: Twelve and a half percent (66/525) of the reviewed infants were born at home. Admitting weights of infants were recorded between 1050-4600g and the mean value was 2891±840 g. The mean admitting age was 8.6 days within a range of 0-30 days. Forty five and a half percent of mothers were illiterate and 9% of fathers were unemployed. During the study period 19.7% out of 66 home-born and 6.5% out of 459 hospital born infants could not be saved. The death reasons among home delivered babies were sepsis (n=2), hemorrhagic disease of the newborn (n=2), pneumonia (n=1), prematurity with asphyxia (n=5), neonatal tetanus (n=1), and congenital anomalies (n=2). Conclusions: The high rate of neonatal deaths remains an important problem for developing countries. Especially home births enhance the burden of the problem. Low literacy rate of women, high rate of unemployment, and working in unqualified jobs are main characteristics of families who prefer home deliveries. Deliveries by trained persons and appropriate perinatal care will significantly reduce incidence of morbidity and mortality in newborns in developing countries.
