Blastocystis Türlerinin Tanısında Yeni Bir Yaklaşım: Direk Floresan Antikor Yöntemi

Özet

Blastocystis dünyada yaygın olarak görülen bağırsak protozoonudur. Çalışmamızda 105 hastaya (erkek/kadın:55/50) ait dışkı örneği nativ-lugol, trikrom, Ringer solüsyonunda kültür ve Direkt Floresan Antikor (DFA, Antibodies Inc., USA) yöntemleri kullanılarak Blastocystis hominis pozitifliği incelenmiş ve bu yöntemler karşılaştırılmıştır. İncelenen örneklerde kültür yöntemi ile Blastocystis protozoonu sıklığı %28.6, bu oran, trikrom boyama yönteminde %15.2, nativ-lugol incelemede %10.5, DFA yönteminde ise %24.8 olarak tespit edilmiştir. Çocuk yaş grubundaki 39 dışkı örneğinin %10.25'inde, erişkin gruptaki 66 örneğin ise %39.4'ünde kültür ile pozitiflik belirlenmiştir. İki yaş grubu arasında Blastocystis varlığı açısından istatisitiksel olarak fark anlamlı bulunmuştur (P<0.001). Kültür yöntemi referans alındığında DFA yöntemi ile korelasyon çok güçlü düzeyde (r=0.858, P<0.001) saptanırken, trikrom boyama (r=0.409, P<0.001) ve nativ-lugol (r=0.403, P<0.01) yöntemleriyle orta düzeyde bir korelasyon belirlenmiştir. DFA yönteminin kültür yöntemine göre duyarlılığı %83.3 özgüllüğü ise %93.7 olarak, trikrom boyama ve nativ-lugol yöntemlerinin duyarlılık ve özgüllükleri ise (sırasıyla %43.3, %78.7 ve %30, %77.6) daha düşük olarak tespit edilmiştir. DFA yönteminin preparat incelemesinin kısa sürede sonuçlandığı, az sayıda protozoon içeren örneklerin incelenmesinde ve de gerek morfolojisi gerekse boyutları nedeniyle protozoonun tanınmasında güçlük yaşanan durumlarda tanı kolaylığı sağlaması açısından yararlı bir yöntem olduğu düşüncesine varılmıştır. Hızlı ve pratik bir yöntem olan DFA yönteminin, uzun sürede sonuçlanan kültür yöntemine alternatif bir tanı yöntemi olabileceği düşünülmektedir

Abstract

Blastocystis is a widely detected intestinal protozoon throughout the world. In our study, stool samples of 105 patients (male/female:55/50) were examined for the Blastocystis hominis positivity using native-lugol exaamination, trichrome staining, culture in the Ringer's solution and Direct Florescent Antibody (DFA, Antibodies Inc., USA) methods and these methods were compared. Blastocystis protozoon frequency was 28.6% in culture, which was 15.2% for the trichrome staining method, 10.5% for native-lugol examination, and 24.8% for DFA method. 10.25% of 39 stool samples of the children and 39.4% of 66 samples of the adults were culture positive. There was statistically significant difference in the presence of Blastocystis between two age groups (P<0.001). Culture taken as reference, the correlation was strong in the DFA method (r=0.858, P<0.001), intermediate in the trichrome staining (r=0.409, P<0.001) and native-lugol (r=0.403, P<0.01). The DFA sensitivity compared to culture was 83.3% and the specificity 93.7%, and the sensitivity and the specificity of the trichrome staining and Native-lugol methods were lower (43.3-78.7% and 30-77.6%, respectively). The conclusion drawn here is that DFA examination requires shorter time and it is useful in lower protozoon density and where the morphology and the dimensions of the protozoon make the diagnosis harder.
