Özet

Amaç: Bu çalışmada, endoskopik yöntemle primer transkanal tip 1 timpanoplasti olan olgularda temporal kas fasyası ve tragal kartilaj perikondriyumun anatomik, fonksiyonel sonuçlarının karşılaştırılması amaçlandı. Yöntem ve Gereçler: Bu çalışma, timpanomeatal fleb elevasyonu yapılmadan gerçekleştirilen transkanal endoskopik tip 1 timpanoplasti olan 80 olgu (48 kadın 32 erkek; ort.yaş 30,2±6,9 yıl aralık 24-52 yıl) ile yapıldı. Çalışmaya alınan olgular, kullanılan greft tipine göre temporal kas fasyası (Grup A) ve tragal kartilaj perikondriumu (Grup B) olmak üzere iki gruba ayrıldı. Ameliyat sonrası 12.aylardaki zarın durumu ve saf ses odyometri sonuçları karşılaştırıldı. Bulgular: Greft tutma başarısı Grup A da %90 iken Grup B %92,5 olarak bulundu. Greft tutma başarısı açısından gruplar arasında istatistiksel olarak anlamlı farklılık yoktu (p=0.412). Grup A ve Grup B arasında preoperatif ve postoperatif hava-kemik aralığı (HKA) değerleri istatistiksel olarak anlamlı farklılık göstermemiştir (p=0.082 ve p=0.242). Grup A ve Grup B'de postoperatif HKA değeri preoperatif döneme göre anlamlı düşüş göstermiştir (p=0.001). Grup A ve Grup B arasında postoperatif HKA kazancı açısından istatistiksel olarak anlamlı fark bulunmamıştır (p=0.108). Sonuç: Endoskopik timpanoplastide tragal kartilaj perikondriyumu ve temporal kas fasya greftlerinin anatomik ve fonksiyonel sonuçlarıbaşarılı bulunmuş olup herhangi birinin diğerine bir üstünlüğü bulunmamıştır.

Abstract

Objective: In the present study, it was aimed to compare the anatomical and functional results of temporal muscle fascia and tragal cartilage perichondrium in patients who underwent primary transcanal endoscopic type 1 tympanoplasty. Materials and Methods: A total of 80 patients (48 females 32 males; median age 30,2 ± 6,9 years interval 24-52 years) who underwent a transcanal endoscopic type 1 tympanoplasty without tympanomeatal flap elevation were enrolled in the study. The patients were divided into two groups as temporal muscle fascia (Group A) and tragal cartilage perichondrium (Group B) according to the type of graft used. The groups were compared according to the results of 12th postoperative pure tone audiometry and to the status of the tympanic membrane. Results: Graft retention success was 90% in group A, while group B was 92,5%. There was no statistically significant differencebetween the groups in terms of graft retention success (p = 0.412). Preoperative and postoperative air-bone gap (ABG) values did not differ statistically between group A and group B (p = 0.082 and p = 0.242). The postoperative ABG value showed a significant reduction in Group A and B compared to its respective value in the pre-operative period (p=0.001). The postoperative gain in the ABG value did not show a significant difference between Group A and B (p = 0.108). Conclusion: The anatomical and functional results of the tragal cartilage perichondrium and temporal muscle fascia grafts in endoscopic tympanoplasty were found to be successful, but neither of them was superior to the other.
