PAROTİDEKTOMİ AMELİYATLARI SONRASI GELİŞEN PARALİZİLERİN SYDNEY FASİAL SKORLAMA SİSTEMİYLE DEĞERLENDİRİLMESİ

Özet
Amaç: Periferik fasiyal paralizi hastalarında paralizinin şiddetini değerlendirmek için çeşitli derecelendirme sistemleri geliştirilmiştir. Bu değerlendirmeler hastayı muayene eden doktorun gözlemiyle yapılmaktadır. Doğru ve standart değerlendirme tanı ve tedavi için önem arz etmektedir. Çalışmamızın amacı parotidektomi yaptığımız hastalarda gelişen fasiyal paralizileri Sydney fasiyal sinir derecelendirme sistemi (SFSDS) ile değerlendirmek ve fasiyal paralizi derecelerini kısa ve orta vadede karşılaştırmaktır. Gereç ve Yöntem: Kliniğimizde parotidektomi yapılan hastaların dosyaları retrospektif olarak tarandı. Hastaların postoperatif 1. gün ve postoperatif 6. ayda SFSDS kullanılarak değerlendirilen fasiyal sinir fonksiyonları kaydedildi. Bulgular: Çalışmaya 24'ü (%70.6) erkek, 10'u (%29.4) kadın olmak üzere 34 hasta dahil edildi. 26 (%76.5) hastaya yüzeyel parotidektomi, 8 (%23.5) hastaya total parotidektomi yapılmıştır. 34 hastanın 15'inde (%44.1) postoperatif 1. günde parezi veya paralizi izlenmiştir. Postoperatif 6. ayda ise 7 (% 20.5) hastada parezi veya paralizi izlenmiştir. Postoperatif 6.ayda 3 hastada hafif sinkinezi (1/3) görülürken 2 hastada orta derecede sinkinezi (2/3) izlenmiştir. SFSDS ile yapılan değerlendirmede postoperatif 1.gün temporal dal, zigomatik dal, bukkal dal, marjinal mandibüler dal ve servikal dalların paralizi skoruyla postoperatif 6. ay paralizi skorları arasında istatiksel olarak anlamlı bir fark saptanmıştır. Sonuç: SFSDS ideal bir fasiyal paralizi derecelendirmesi yaptığından kullanımı giderek yaygınlaşmaktadır. Parotidektomi sonrasıgelişen fasiyal paralizlerin değerlendirilmesi, izlenmesi ve raporlanmasında güvenle kullanılabilinir.
Abstract
Objective: Various rating systems have been developed to evaluate the severity of paralysis in peripheral facial paralysis patients. These evaluations are made with the observation of the doctor who examined the patient. Accurate and standard evaluation is important for diagnosis and treatment. The aim of our study is to evaluate facial paralysis, developing in patients undergoing parotidectomy with the Sydney facial nerve rating system (SFNRS) and to compare facial paralysis degrees in the short and medium term. Material and Method: The files of patients who underwent parotidectomy in our clinic were reviewed retrospectively. Facial nerve functions evaluated using SFNRS were recorded on the postoperative 1st day and postoperative 6th month. Results: 34 patients, 24 (70.6%) male and 10 (29.4%) female, were included in the study. Superficial parotidectomy was performed in 26 (76.5%) patients and total parotidectomy was performed in 8 (23.5%) patients. In 15 (34.1%) of 34 patients, paresis or paralysis was observed on the first postoperative day. In the postoperative 6th month, paresis or paralysis was observed in 7 (% 20.5) patients. In the postoperative 6th month, 3 patients had mild synkinesia (1/3), while 2 patients had moderate synkinesia (2/3). In the evaluation made with SFNRS, a statistically significant difference was found between postoperative 1st day temporal branch, zygomatic branch, buccal branch, marginal mandibular branch and cervical branch paralysis score and postoperative 6th month paralysis scores. Conclusion: As SFNRS makes an ideal facial paralysis rating, its usage is becoming more and more common. It can be used safely in the evaluation, monitoring and reporting of facial paralysis developing after parotidectomy.
