Büyük basit böbrek kisti zemininde saklı böbrek hücreli karsinom: Olgu sunumu

Özet
Böbrek içinde herhangi bir dilate kanal veya tübül renal kist olarak isimlendirilir. Konjenital ve edinsel renal kistik hastalık nefron ve toplayıcı kanal- lardan gelişir. Kistler böbrek içinde yaygın biçimde veya sadece bir segmente, unilateral ya da bilateral olarak görülebilir. Basit kistlerde tanı kolayca konulabilir. En önemli tanısal sorun, maligniteye yatkınlık gösteren kompleks kistlerle, basit kistleri doğru biçimde ayırt etmektir. Malignite şüphesi oluşturan durumlar kalınlaşmış, kalsifiye, irregüler septalar ve kontrast tutulumudur. Basit renal kist zemininde gelişen renal hücreli karsinom ( RCC) ile ilgili litaratürde oldukça az sayıda vaka rapor edilmiştir. Biz bu olgumuzda sol böbrek alt polünden abdominal pelvisi dolduracak kadar büyük basit kist zemininde gelişen renal hücreli karsinomayı sunmayı amaçladık. Herhangi bir ek hastalığı bulunmayan karnın sol üst ve alt kısmında bombeleşme şikâyeti olan 36 yaşında bayan hasta kliniğimizde değerlendirildi. Bosniak Tip 2 kistik lezyon tanısıyla, laporaskopik kist eksizyonu operasyonu yapıldı postoperatif patoloji raporu RCC geldi. Bu doğrultuda hastamıza tamamlayıcı cerrahi olarak radikal nefrektomi yapıldı. Bu olguda basit renal kistlerde de RCCnin oluşabileceğini fark etmenin önemini vurgulamaktadır. Bu nedenle atipik bulgular varlığında ileri araştırma muhakkak yapılmalıdır. Bütün ileri radyolojik tetkiklere rağmen kist RCC li vakamız tanısı preop konamadı. RCC tanısı konmuş vakalarda patolojinin şüphesi varsa mutlaka radikal nefrektomi yapılmalıdır.
Abstract
The tubule of the kidney or renal cyst is called a dilated duct. Congenital and acquired renal cystic disease develops in the nephron and collect- ing channels. Renal cysts are common in the way or as a segment, can be seen as unilateral or bilateral. In simple cysts, the diagnosis can easily be made. The most important diagnostic problem, with a tendency to malignancy,of complex cysts,and simple cysts, is to distinguish the correct format. Suspicion of malignancy are thickened, calcified, irregular septa and contrast fixation. Simple renal cysts with underlying renal cell carcinoma (RCC) in a relatively small number of cases have been reported in the literature. We aimed to present this caseof a 36-year-old female patient in our clinic, evaluated with cambered complaint Bosniak type 2 diagnosis of cystic lesions. The lower pole of the left renal pelvis was large enough to fill a simple abdominal cyst with underlying renal cell carcinoma, without any additional disease in the upper left abdomen and lower part of the kidney. Excision of the cyst was carried out and the laparascopic postoperative pathology reported a RCC. In our patient an integral surgical radical nephrectomy was performed. This case highlights the importance of recognizing simple renal cysts which may form in the RCC. Therefore, further research should be considered in the presence of atypical findings. Although no cases of cystic RCC, with all advanced radiological examinations, could not be preopera- tively diagnosed, in RCC cases diagnosed with suspected pathology, radical nephrectomy should be done.
