Özet

Üriner sistem taş hastalığı toplumda sık gözlenen bir sağlık sorunudur. Son 2 dekatta; perkütan nefrolitotomi (PNL), vücut dışı şok dalga litotripsisi (ESWL) ve retrograd intrarenal cerrahi (RIRC) gibi minimal invaziv yöntemler popüler olmuştur. Avrupa Üroloji Derneği'nin 2014 üriner sistem taş hastalığı kılavuzlarında, 20 mm üstündeki böbrek taşlarına 1. basamak tedavi olarak PNL, 10 mm altındaki taşlara ESWL önerilmektedir. 10 ile 20 mm arasında taşlarda ise taşın yerleştiği lokalizasyon, taş cinsi, kaliks anatomisi ve hastaya ait faktörler devreye girmektedir. RIRC, holmium YAG lazer ile birlikte taş tedavisinde etkin ve güvenilir bir yöntemdir. Başlangıçta böbrek alt polünde birden fazla taş, ESWL başarısızlığı, komorbid hastalık varlığı gibi daha dar bir olgu grubunda kullanılırken; cerrahi tecrübenin gelişmesi üreteroskopların çaplarının küçülmesi, daha iyi görüş sağlayan optiklerin geliştirilmesi ve ekipmanın çeşitlenmesi sonucu günümüzde diğer minimal invaziv yöntemlere alternatif bir tedavi modalitesi haline gelmiştir. Özellikle alt pol taşlarında; daha yüksek tam taşsızlık oranları ve daha az sayıda ek cerrahi girişim gerekliği ile ESWL'ye üstünlüğü gösterilmiştir. PNL ile karşılaştırmalı metaanaliz çalışmalarında ise, PNL'nin tam taşsızlık oranları daha yüksek saptanmaktadır. Kısa hastanede kalış süresi, kısa ameliyat zamanı RIRC yönteminin avantajı olarak belirtilebilir. Gelecekte fleksibl alet ve cerrahi ekipman teknolojisi geliştikçe, RIRC kullanılan olgu çeşitliliğinin artacağına inanılmaktadır

Abstract

Urinary stone disease is a common health problem in the community. In the last two decades, percutaneous nephrolithotomy (PNL), extracorporeal shock wave lithotripsy (ESWL), and retrograde intrarenal surgery (RIRS) have become popular as minimally invasive methods. The European Association of Urology 2014 urinary stones guideline recommends PNL as the first line of treatment for kidney stones above 20 mm. ESWL remains the first choice for stones of <1 cm within the renal pelvis and upper or middle calices. For kidney stones between 10 to 20 mm, the location and movement of the stone, stone type, calyx anatomy, and patients status contribute to the choice of treatment. RIRS in combination with holmium YAG laser is effective and safe. Initially, RIRS was offered for was lower calyceal stones, multiple stones, post-ESWL failure, comorbid illnesses, reduction in the diameter of development of surgical experience ureteroscopy, the development of optics providing better visibility, and the diversification of the equipment. At present, RIRS has become an alternative treatment modality to other minimally invasive therapy. It is thought that advances in technology will lead to more flexible instruments and surgical equipment for RIRS therapy
