Otosklerozda cerrahi sonuçlarımız

Özet
Otoskleroz kemik labirent ve stapesin primer, distrofik bir hastalığıdır. Otosklerotik odağın görüldüğü yere ve derecesine göre olguların bazıları asemptomatik olabildiği gibi bazılarında iletim tipi, sensorinöral tip veya mixt tip işitme kaybı olabilir. Bu çalışmaya, Süleyman Demirel Üniversitesi Tıp Fakültesi KBB Anabilim Dalı'nda Mayıs 1996-Mayıs 1999 tarihleri arasında klinik ve odyolojik tetkikler sonucu otoskleroz tanısı ile stapedotomi+protez tatbiki yapılan 10 hasta dahil edildi. Olgularımızın 2'si (%20) erkek, 8'i (%80) kadın idi. Yaşları 25-48 arasında değişmekte (ortalama 36.7±7.8) idi. Olguların 5'inde (%50) unilateral, 5'inde (%50) bilateral otoskleroz saptandı. Bir olguda 0.4 mm çapında teflon piston protez, 9 olguda 0.4 mm çapında platin-teflon protez kullanıldı. Olgularımızın preoperatif pure ton odyometrilerinde 0.5-2 kHz'de hava yolu iletimi ortalaması 53.1(9.5 dB, hava-kemik aralığı ortalaması 30.0(9.2 dB idi. Altı (% 60) olguda preoperatif tinnitus şikayeti vardı. Postoperatif 6'ncı ayda hava yolu iletimi ortalaması 38.2±11.8 dB, hava-kemik aralığı ortalaması 16.8±7.1 dB olarak tespit edildi. Cerrahi sonrası 4 (% 67) olguda tinnitus şikayeti düzeldi. Vaka sayımızın az olması ile beraber otosklerozda stapedetomi + küçük çaplı platin- teflon piston tatbiki ile işitme ve tinnutus yönünden kısa dönemde iyi sonuçlar aldığımızı söyleyebiliriz. Olgularımızın uzun dönem sonuçlarını verebilmek için zamana ihtiyacımız vardır ve daha detaylı sonuçlar rapor edebilmemiz içinde vaka sayımızın artması gerekmektedir.
Abstract
Otosclerosis is a primary and dystrophic disease of bony labyrinth and stapes. Some of the cases may be asymptomatic or some may have conductive, sensorineural or mixed type hearing loss, with respect to the location and degree of the otosclerotic focus. Ten cases with otosclerosis who underwent stapedotomy in Süleyman Demirel University, School of Medicine, ENT Department between May 1996-May 1999 were included into this study. Two (20%) of the cases were male, 8 (80%) were female. The ages range between 25-48 (36.7±7.8). Otosclerosis was detected unilaterally in 5 (50%) cases and bilaterally 5 (50%) cases. The mean of air conduction threshold and air-bone gap were 53.1 ±9.5 dB and 30.0+9.2, respectively at 0.5-2kHz. The mean of air conduction threshold and air-bone gap were 38.2±11.8 dB, 16,8±7.1dB, respectively in the postoperative 6th month. We consider better hearing results could be obtained by stapedotomy and application of small diameter teflon platinum piston though the number of our patients is inadequate to assess. In order to provide long-term outcomes, time is of critical importance and it is necessary to increase number of the patients to analyze the results in detail.
