Özet
Guillain-Barre Sendromu, bir üst solunum yolu veya gastrointestinal sistem hastalığını takiben birkaç hafta içinde gelişen, genellikle bacaklarda başlayan kas güçsüzlüğü, hipotoni ve hiporefleksi ile kendini gösteren, beyin omurilik sıvısında albumino-sitolojik dissosiyasyon bulunan ve elektromiyografîde motor ileti hızında azalma ile karakterize segmental demiyelinizasyon ile giden bir akut polinöropatidir. Bu olgu sunumunda, parotis şişliğinin yedinci gününde belirti veren kabakulak infeksiyonuna bağlı hızlı gelişen bir Guillain-Barre Sendromu bildirilmiştir. Olgu, 4 yaşında kız çocuğu, ateş, sol kulak altı ve önünde şişlik, yürüyememe yakınmaları ile başvurdu. İlk fizik muayenesinde dikkat çekici bulgular, sol parotis bölgesinde şişlik, alt ekstremitelerde kas güçsüzlüğü ve hipotoni, kemik-veter reflekslerinde azalma. Değerlendirme amacıyla yapılan lomber ponksiyonda; hücre yok, protein 347 mg/dl, serumda kabakulak IgM pozitif ve EMG'de motor ileti hızında yavaşlama saptandı. 5 günlük intravenöz immunglobulin tedavisi ile sekelsiz iyileşti.
Abstract
The Guillain Barre Syndrome (GBS) is an acute polyneuropathy resulting from segmented demyelination of peripheral nerves. It usually occurs several weeks after an upper respiratory or gastrointestinal tract illness. It is characterized by progressive motor weakness usually in lower extremities, hypotonia, hyporeflexia and minor sensory disturbances. Supporting laboratory festures are elevation of the cerebrospinal fluid protein without appearance of cells called albuminocytologic dissociation and reduction in motor conduction velocity. We present a case of Guillain Barre Syndrome developed 7 days after mumps infection. The affected child was a four years old girl who presented with fever, swelling of the left side of her face and motor weakness in lower extremities. There were swelling of the left parotis gland, hypotonia, decreased deep tendon reflexes on physical examination. Increased CSF protein (347 mg/dl) with no cell presence of IgM antibody against mumps virus and decreased nerve conduction velocity in EMG were other supporting laboratory findings. The child became completely well with five days TVIG therapy.
