Özet
Outpatient hastalarda uygulanan artroskopik cerrahilerde, intraartiküler, artikain ile artikain + morfin uygulamasını karşılaştırdık. ASA I-II grubu hastalar randomize olarak 20 kişilik iki gruba ayrıldı.Her iki gruba sedasyon amacıyla 100 µ fentanyl-5 mg dehidrobenz peridol IV verildi.Hiçbir hastaya turnike uygulanmadı. Lokal anestezik ajan olarak intraartiküler anestezide daha önce kullanılmamış olan, hızlı etki eden ve etki süresi uzun olan artikain tercih edildi. Epinefrinli artikain formu ile hem intraartiküler kanama engellenip net görüntü elde edildi hem de artikainin emilimi yavaşlatılıp, toksik kan seviyelerine çıkması önlendi. I.gruba artikain (epinefrin) II.gruba artikain (epinefrin) morfin sülfat verildi. Hastalar perop ve postop 1,2,3,4,5,6. saatlerde izlendi. İki grupta da Kan Basıncı, Kalp Atım Hızı, Arteriyel Oksijen Satürasyonu değerleri arasında anlamlı fark görülmedi. Her iki grupta,diğer anestezi metodlarına kıyasla intraartiküler anestezi metodu ile hastaların memnuniyet derecelerinin yüksek.ameliyathanede kalış sürelerinin daha kısa turnike kullanılmaması nedeniyle mobilizasyon zamanlarının daha erken olduğu ve hastalara daha ucuza mal olduğu görüldü. Postoperatif analjezi süresi açısından karşılaştırıldığında; I.grupta bu süre ortalama 3.10 saat iken II.grupta ortalama 5.65 saat olarak bulundu. Sonuç olarak intraartiküler artikain + morfin kombinasyonunun tek başına artikain kullanılmasına göre daha uzun süre postoperatif analjezi sağladığı saptandı.
Abstract
In this Study, we compared Articain to articain + morphine in 2 groups of patients with knee-joint problems. These patients with ASA I,II who underwent arthroscopic examination were divided randomly in to 2 groups, each consisting of 20 patients. 100 µ of fentanyl and 5 mg dehidrobenzperidol were applied IV to each group of patients to provide sedation and decrease the pain due to extraarticular stretchings. No tourniquets were applied to the patients. Articain, which was previously used for the intraarticular anesthesia, was chosen for the local anesthesia because of its rapid and long duration of action. Its combination with epinefrine provided us a better view, less intraarticular bleeding and a decreased absorbtion rate which prevented the patients from reaching toxic blood levels. The first group of the patients were treated with articain (epinephrine) and the second group with articain (epinephrine) + morphine. Patients in each group were observed every how during 6 hrs period pre and postoperatively. No meaningful difference was encountered amongst Blood Pressure, Heart Rate, O2 saturation rates. In each group of patients treated with intraarticular anesthesia compared to other methods of anesthesia, we noticed that the patients were more satisfied, the costs were less the duration of operation and the period of mobilization were shorter. When we compared the duration of analgesia during the postoperative period, the first group had 3.10 hrs as an average timen of anesthesia. Whereas second group had 5.65 hrs. As a result, combined use of articain with morphine provided longer postoperative analgesic compared to articaine alone.
