Asit ayırıcı tanısında serum-asit albumin farkının asit total kolesterol düzeyleri ile karşılaştırılması

Özet
Asit ayırıcı tanısında standart test olarak serum-asit albumin farkı kullanılmaktadır. Asit total kolesterol düzeyi ölçümleri de son yıllarda aynı amaçla kullanılmaya başlanmıştır. Çalışmamızda portal hipertansiyon ile birlikte olan ve olmayan olarak iki gruba ayırdığımız asitli hastaların ayırıcı tanısında serum asit-albumin farkı ve asit total kolesterol düzeyi ölçümlerinin duyarlılık, özgüllük ve doğruluk derecelerine bakılarak karşılaştırılması amaçlanmıştır. Bu amaçla portal hipertansiyonu olan 4 kronik hepatit 32 karaciğer sirozu ve bir Budd-Chiari Sendromu tanısı almış hasta ile portal hipertansiyonu olmayan 5 malign asitli ve 6 tüberküloz peritonitli toplam 48 hastada, serum-asit albumin farkı ve asit total kolesterol düzeyleri karşılaştırıldı. Serum-asit albumin farkı değerinin portal hipertansiyonu olan ve olmayan asitli hastaların ayırıcı tamsındaki duyarlılığı % 95, özgüllüğü % 64, doğruluğu % 88 olarak saptanmıştır. Asit total kolesterol düzeyinin ayırıcı tanıdaki duyarlılığı ise % 95, özgüllüğü % 55, doğruluğu % 85 bulunmuştur. Sonuç olarak serum-asit albumin farkı ve asit total kolesterol düzeyi portal hipertansiyonu olan ve olmayan hastalardaki asit ayırıcı tanısında oldukça duyarlı iken özgüllük ve doğruluk açısından yeteri kadar tatmin edici bulunamamıştır.
Abstract
In ascites differential diagnosis, serum-ascites albumine difference is used as standard test. Ascites total cholesterol level countings have been used in the last years too. In our study, we divided our patients who had ascites into two groups having portal hypertension or not and intheir differential diagnosis, we aimed to compare serum-ascites albumine difference and ascites total cholesterol levels countings by using spesivity, sensitivity and accuracy levels. For this aim, we compared serum-ascites albumine difference and ascites total cholesterol levels in 4 cronic hepatitis 32 liver cirrhosis patients who had portal hypertension and one patient who had the diagnosis at Budd-Chiari Syndrome with 5 malign ascitesed patients and 6 tuberculosis peritonitis patients who did not have portal hypertension. The sensitivity of serum ascites albumine difference value is %95, specifity of that is %64 and accuracy of that is %88 in the differential diagnosis of patients who had ascites with or without portal hypertension. In differential diagnosis. The sensitivity of ascites total cholesterol level is %95, speciflty of that is %55, accuracy of that is %85. As conclusion, in the differential diagnosis of patients who had ascites, serum-ascites albumine difference and ascites total cholesterol levels in patients who have portal hypertension or not are sensitive enough butthey are not satisfying according to their specifity and accuracy.
