Özet

Herpes Zoster Sefalikus, Varicella Zoster virüsünün VII. kraniyal sinir paralizisi (Ramsay-Hunt) ile birlikte diğer kraniyal sinirleri de tutmasıdır. Kliniğimizde ender rastlanan herpes zoster sefalikus tablosu olan 85 yaşında erkek hasta izlendi. Hastada aurikular ve preaurikular bölgede herpetik veziküller, fasiyal paralizi ve fasiyal ağrı ile başlayan tabloya işitme kaybı, tinnitus (VIII. kraniyal sinir), yüzün aynı tarafında his kaybı (V. kraniyal sinir), yutma güçlüğü (IX. kraniyal sinir), omuz düşüklüğü ve tortikolis bulguları eklendi. Hastaya 3. günden itibaren prednisolon 40 mg/gün I.V., acyclovir 750 mg/gün I.V. uygulandı. Hastanın 2. haftaya kadar progresyon gösteren klinik tablosu 2. haftadan itibaren önce stabil hale gelip sonra düzeldi.

Abstract

In the syndrome of Herpes Zoster Cephalicus, Varicella Zoster virus infects the seventh cranial nerve (Ramsay-Hunt), as well as other cranial nerves. in our clinic we followed a patient, 85 years old, with the rare syndrome of Herpes Zoster Cephalicus. The patient had herpetic vesicules on the auricular and preauricular region, facial paralysis facial pain and loss of hearing with tinnitus (VII. and VIII. cranial nerve), loss of sensation on the same side of the face (V. cranial nerve), difficulty with swallowing, (XI. cranial nerve), drop of shoulder with torticollis (XI. cranial nerve) were added to the signs and symptoms. On the third day after the onset of the symptoms medical therapy with I.V. prednisolone 40 mg/day and I.V. acyclovir 750 mg/day was started. The patient's clinical picture worsened till the end of the second week. After that the symptoms stabilised and the patient recovered.
