Özet
Servikal tüberküloz lenfadenit Türkiye'de güncelliğini koruyan bir problemdir. Geleneksel tanı metodlarının yanı sıra yeni yöntemlerin ortaya çıkmasına karşın tanısında zorluklarla karşılaşılmaktadır. Boyunda kitle dışında herhangi bir bulgusu olmayan 26 tüberküloz lenfadenitli hasta kliniğimizde retrospektif olarak tetkik edilmiştir. Hastaların hepsinde akciğer grafisi normaldi. Purifiye protein derivesi (PPD) deri testi, uygulanan bütün hastalarda pozitifti. Sedimentasyon genel olarak yüksek, formülde nisbi lenfositoz mevcuttu. Ultrasonografik ve komputerize tomografi tetkiklerinde hipoekoik lezyon ve kistik yapı saptandı. İ.İ.A.B.(İnce İğne Aspirasyon Biyopsisi) uygulanan 6 hastanın 3 ünde tüberküloz ile uyumlu bulgular saptandı. Eksizyonel biopsi sonrasında patolojik tetkik ile kesin teşhis konuldu. Bu sonuçlar ışığında, PPD ve eksizyonel biopsi servikal tüberküloz lenfadenit teşhisinde en değerli yöntemler olarak görülmektedir.
Abstract
Cervical tuberculous lymphadenitis is still a recent problem in Turkey. Despite the new diagnostic methods were added to classical methods, there are difficulties in diagnosis. in our clinic, 26 patients with cervical tuberculous lymphadenitis were investigated retrospectively and except the neck mass there was no another sign in these patients. In all cases, X-Ray chest radiograms were normal. Purified protein derivative (PPD) skin test was positive in all of the patients in which the test were applied. Erythrocyte sedimentation rate was usually rised and lymphositosis was obtained. Ultrasound and CT seans showed hipoechoic and cystic lesions. Three of six patients which were made fine needle aspiration biopsy gave tuberculosis like findings After exicional biopsy and histological examinations the certain diagnosis was revealed. in the light of these results, PPD and exicional biopsy were seen the most valuable methods in the diagnosis of cervical tuberculous lymphadenitis.
