Kronik Vajinal Akıntılı Hastalarda HPV-DNA Pozitiflik Oranı ve Sitopatolojik Sonuçların Değerlendirilmesi

Özet

Amaç: Kronik enfeksiyonun HPV-DNA persistansı ve displazik değişimleri kolaylaştırdığı yönünde yeni yayınlar mevcuttur. Kronik vajinal akıntılı hastalarda bu beraberliğin sıklığı, servikal sitolojik ve histopatolojik inceleme sonuçları araştırıldı. Yöntemler: Kronik mukopürülan akıntı nedeniyle en az üç kez vajinit tedavisi görmüş, HPV DNA'sına ulaşılan 103 hasta retrospektif olarak incelendi. HPV DNA pozitifliği için risk faktörleri değerlendirildi. Hastalar HPV DNA varlığı ve yokluğuna göre iki gruba ayrıldı (HPV DNA pozitif ve negatif). Sitopatolojik ve histopatolojik inceleme sonuçları ve risk faktörleri bu iki grup arasında karşılaştırıldı. Bulgular: Hastaların yaş ortalaması 35±9 idi. Yüzde 64 hastada HPV-DNA pozitifti. HPV-DNA'sı pozitif olan hastaların %41'inde anormal sitolojik bulgular, %16'sında CIN saptandı. HPV-DNA'sı negatif grubun %16'sında anormal sitolojik bulgular saptanırken, ikisinde CIN saptandı. HPV-DNA pozitif ve negatif gruplar arasında servikal sitolojik anormallik ve CIN saptanması açısından istatistiksel olarak anlamlı fark saptanmadı. Sonuç: Çalışmamızda kronik mukopürülan akıntılı hastalarda HPV DNA pozitifliği ve anormal sitolojik ve histolojik bulguları genel literatüre göre daha yüksek oranda saptadık

Abstract

Aim: There are new publications stating that chronic infection may facilitate dysplastic changes and persistence of human papillomavirus (HPV)-DNA positivity. Frequency of this coincidence and cervical cytological and pathological examination results were investigated in patients with chronic vaginal discharge.Methods: One hundred three patients, who were treated at least 3 times for chronic mucopurulent discharge with accessible HPV DNA results, were analyzed retrospectively. Risk factors were identified for HPV DNA positivity. HPV DNA positive-patients were divided into two groups (positive and negative) according to the presence and absence of HPV DNA. The results of cytopatological and histopathological examinations and risk factors were compared between the two groups.Results: The mean age of the patients was 35±9 years. Sixty-four percent of the patients were positive for HPV DNA. Of the HPV DNA positive patients, 41% had abnormal cytological findings, 16% had cervical intraepithelial neoplasia (CIN). Of the HPV DNAnegative patients, 16% had abnormal cytological findings and, 2 patients had CIN. There was no statistically significant difference in cervical cytological abnormalities and CIN between HPV-DNA positive and negative groups.Conclusion: In our study, the rates of HPV DNA positivity and abnormal cytopathological results among patients with chronic vaginal discharge were found to be higher than that reported in the literature
