Özet
Çalışmamızda inhalasyon ajanı sevofloran ve intravenöz ajan propofolün indüksiyon ve derlenme özelliklerini karşılaştırmayı amaçladık. Etik kurul onayı alındıktan sonra çalışma, ASA I-II sınıfına dahil, yaşlan 25-64 arasında değişen ve histerektomi operasyonu planlanan 55 hasta üzerinde gerçekleştirildi. Hastalar rasgele iki gruba ayrıldı. Propofol grubunda( Gl); 1-1.5 ug/kg fentanil, 2mg/kg propofol ile intravenöz indüksiyonu takiben, idamede propofol perfüzyonu ile birlikte %50-50 N2O/O2; sevofloran grubunda(G2) indüksiyonda; 1-1.5 ug/kg fentanil (iv), maske ventilasyon uygulanarak verilen sevofloram (%2-6) takiben idamede sevofloran (1,5MAC) ile birlikte %50-50 N2O/O2 kullanıldı. Her iki grupta hastaların indüksiyon, ekstübasyon, göz açma, kooperasyon süreleri ölçüldü ve indüksiyondaki yan etkileri değerlendirildi. Hastalann preoperatif dönemde, indüksiyonda, cerrahi keşi sırasında, cerrahi dönem boyunca 15 dakikada bir, ekstübasyonda SAB, DAB, OAB, SpO2 kaydedildi: Sevofloran grubunda, kirpik refleksi kaybı süresi (63,39 ± 8,30) sn, propofol grubuna (39,52 ± 6,08) sn göre anlamlı derecede daha uzundu (p 0.05). Sonuç olarak propofolün hızlı indüksiyon ve derlenme sağlaması nedeniyle anestezi indüksiyon ve idamesi için uygun bir ajan olduğunu söyleyebiliriz.
Abstract
In this study we aimed to compare the induction and recovery characteristics and haemodynamic effects of sevoflurane (the inhalation anaesthesic) and propofol (the i.v anaesthesia). After ethics committee approval, 55 patients (ASA I- ASAH categories), aged between 25 - 64, prepared for hysterectomy operation were randomly allocated into two groups. Group I received 1-1,5 fig/kg fentanyl, 2 mglkg propofol i.V. for the induction of anaesthesia and propofol perfusion with 50150% - N2OI02 has been applied for the maintenance of anaesthesia. Group II received l-1.5uglkg fentanyl i.v. and 2 -6 % sevoflurane via mask for the induction of anaesthesia and received sevoflurane (1.5 MAC) with 50150% N2O I 02 for the maintenance of anaesthesia. In both groups, the induction, extubation, eye-opening and cooperation times were recorded. The side effects of the both drugs were detected and recorded. SAP, DAP, MAP and SP02 were recorded preoperatively, during induction, at the incision, peroperatively, during extubation. The time for loosing eyelash reflex in group II (63.39 ± 8.30sec) was significantly longer than group I (39.52 ± 6.08sec) (p<0.05). The extubation, eye-opening and verbal response times of group II (5.92 ± 0.54min, 7.00 ± 0.56min, 7.9±0.56min) were significantly longer than group I (4.61 ± 0.54min, 5.28 ± 0.54min, 6.44 ± 0.51min) (p<0.05). There were no significant difference between groups regarding the complications and the haemodynamic values (p>0.05). In conclusion, propofol appears to be a suitable agent for induction and maintenance of anaesthesia because of shorter induction time and early recovery time.
