Çocuklarda postoperatif bulantı ve kusmanpn profilaksisinde "Tropisetron" kullanılması

Özet

Çocuklarda tonsillektomi sonucu bulantı ve kusma sık görülen bir problemdir. Pediatrik onkolojide,kemoterapi sırasında kullanılan ve yeni bir 5-HT3 reseptör antogonisti olan tropisetronun, tek doz uygulamadan sonraki etkinliğini ve güvenirliliğini, postoperatif yüksek kusma riski taşıyan çocuklarda araştırdık.Tonsillektomi veya adenotonsillektomi olacak yaşları 2-12 arası 58 çocuk çalışmaya alındı.Hastalar randomize olarak aynldı.Çalışmamız plasebo kontrollü ve çift kör olacak şekilde planlandı.Hastalara indüksiyondan hemen sonra 0,1 mg/kg (0,lml=0,lmg) tropisetron veya plasebo, tek doz olarak verildi.Anestezi idamesi opiod ve kas gevşetici kullanılmadan, sevofluran ve N2O/Û2 ile sürdürüldü.Vital bulgular, sedasyon derinliği, postop kusmanın meydana gelmesi ve istenmeyen etkiler kaydedildi.Tropisetron grubunda çocukların % 65,3'ü postop kusma göstermedi.Buna karşın plasebo grubunda %34,7 kusmaya rastlandı (P=0,0024).Tropisetron grubunda 3'ten fazla postop kusma atağı sadece % 10,2 iken, kontrol grubunda % 22,4 bulundu (P=0.0004).Ek antiemetik ihtiyacı tropisetron grubunda %10,4 iken, kontrol grubunda anlamlı derecede yüksek olan %28,6 bulundu(P=0.025).Çalışmada önemli yan etkiler görülmedi. Tropisetron'un profilaktik intravenöz tek dozu tonsillektomi veya adenotonsillektomi sonrası ilk 24 saatte çocuklarda kusmada etkili bulunmuştur.Yan etkilerin insidansının düşük olması nedeniyle tonsillektomi sonrası çocuklarda tropisetron'un profilaktik olarak güvenli bir ajan olduğu kanısına vardık.

Abstract

Postoperative nausea and vomiting (PONV) after tonsillectomy is a common problem in children. Tropisetron is a new 5-HT3 receptor antagonist and is successfully used in pediatric patients receiving cancer therapy. The aim of the study was to assess efficacy and safety of a single intravenous dose of tropisetron for prevention of PONV in pediatric patients at risk for postoperative vomiting. In a randomised, double-blind, placebocontrolled trial, we studied 58 children aged 2-12 years undergoing tonsillectomy or adenotonsillectomy. Patients received placebo or tropisetron 0,lmg(=0,lml)/kg body weight immediately after induction of anaesthesia. A standart general anaesthetic technique (sevoflurane I N2OI02) without neuromuscular blockers or opioids) was used. Perioperative vital signs, grade of sedation and episodes of postoperative nausea and vomiting were recorder. No vomiting episodes occurred in 65,3% of the tropisetron treated patients compared to 34,7% of the placebo group (p=0,0024).0nly 10,2% of the tropisetron treated patients vomited more than 3 times compared to 22,4% of the control patients (p=0,025).No significantly lower in the study group (10,45) compared to 28,6% (p=0,025).No significant adverse effects of the study medication were shown A single intravenous prophylactic dose of tropisetron effectively reduces the incidence of PONV during the first 24 postoperative hours after tonsillectomy and/or adenoictomy. Because of the low incidence of adverse effects, the prophylactic use of tropisetron seems to be safe and justified inpaediatric surgical patients at high risk for postoperative vomiting.
