Türkiye' de sığır ve koyunlarda brucellosis' in seroepidemiyolojisi

Özet

Brucellosis dünyanın her yerinde rastlanan en yaygın zoonozlardan biri kabul edilmektedir. Ülkemizde 1984 yılında, ülkesel bir kontrol ve eradikasyon projesi yürürlüğe konulmuştur. Projenin 26 yıl sürmesi planlanmıştır. Bu projeye göre, Türkiye 5 bölgeye ayrılmış ve bu bölgelerde bütün 4-8 aylık dişi danalar Brucella abortus S-19 ile aşılanacak ve kulak küpesi takılarak kayıt altına alınacak ve bütün kuzu ve oğlaklar Brucella melitensis Rev-1 aşısı ile aşılanacaklardır.1989 yılında ülke çapında yapılan serolojik survey sonuçlarına göre, bölgelere göre değişen %0-10 oranındaki hastalık prevalaıısı, sığırlarda %3.56 ve koyunlarda %1.26 olarak, 1990 yılında ise %1.2 ve %2.08, 1991 yılında yapılan son ülke çapındaki sero surveyde ise sığırlarda %1.01 ve koyunlarda %1.83 olarak tespit edilmiştir. Trakya bölgesinde 1991 ve 1993 yılları arasında bütün ergin sığır ve koyunlar düşük doz Brucella aşıları ile aşılanmışlar. Bu pilot çalışmadan sonra düşük doz aşılama ülkesel kontrol ve eradikasyon projesine dahil edilmiştir.Koruma ve Kontrol Genel Müdürlüğü'nün isteği doğrultusunda 1996 yılında Türkiye çapında brucellosis yönünden bir sero-sürvey yapılmasına karar verilmiş ve 1997 yılında proje başlamıştır.Bu çalışmada, ülke çapında her ilin dört ilçesinden tesadüfi örneklemeye göre 34458 sığır ve 30433 koyundan olmak üzere toplam 64891 adet serum örneği toplandı ve bütün serumlar tarama testi olarak rose bengal plate testi (RBPT) ile tarandıktan sonra pozitif serumlar doğrulama testi olarak komplement fıkzasyon (CFT) ile test edildiler.Brucellosis prevalansı sığır populasyonunda %1.43, koyun populasyoııunda ise % l .97 olarak tespit edildi. Brucellosis'in sürü prevalansını belirlemek için her ilçenin dörder köyü seçildi ve 1313 sığır sürüsünde % 11.4 ve 1077 koyun sürüsünde %15 olarak tespit edildi. Yapılan bu çalışma, Türkiye'de 1984 de başlayan 14-15 sene gibi uzun bir aşama geçiren "Türkiye Brucellosis Mücadele Projesi"inde aksamalar olduğunu vurgulamaktadır. Alınan sonuçlara göre aşılama çalışmalarına daha titizlikle ve önemsenerek devam edilmesi gerekmektedir.

Abstract

Brucellosis is considered one of the most important zoonoses in the world. In our country, the national control and eradication project has been put into practice since the beginning of 1984. This project has been planned to be completed in 26 years. According to the plan of the project, Turkey was divided into 5 regions and according to the plan of the operation, all female calves of 4-8 months old are to be vaccinated with Brucella abortus S-19 vaccine and vaccinated animals are to be ear tagged and registered. All lambs and kids are to be vaccinated with Brucella melitensis Rev-1 vaccine.According to the national survey performed in 1989, the prevalance rates of infection in cattle was 3.56% and in sheep 1.26%. The rate range varied region to region from 0% to 10%. In 1990, the prevalence rates in cattle and sheep were 1.2% and 2.08%, respectively. Last sérologie survey was carried out in the country in 1991. In this serosurvey, the seroprevalance of infection was determined as 1.01% in cattle and 1.83% in sheep. In Thrace region, adult cattle and sheep were vaccinated with reduced dose Bruvella vaccines from 1991 to 1993. After this pilot project, reduced dose vaccination was added to the program of the national control and earication project.General Directoires of Protection and Control decided to cany out of a sero-survey for Brucellosis in Turkey in 1996 and this project was programmed to begin in 1997.A total 64891 serum samples, of which 34458 from cattle and 30433 from sheep, were collected randomly from four districts of each province of the country. All serum samples were tested with Rose Bengal Plate Test (RBPT) as a screening test and positive sera were tested with Complement Fixation Test (CFT) as a confirmatory (definitive) test. Brucellosis prevalenci was found as 1.43% for cattle and 1.97% for sheep population. To investigate herd prevalance of brucellosis, 1313 cattle herd and 1077 sheep herd were selected from four villages of each district of the whole country. The herd seroprevalance was 11.4% in cattle herd and 15% in sheep herd.This study clearly shows that there are some hitching points in the "Turkey Brucellosis Fighting Project", althoungh this project has been implemented for 14-15 years. According to the results obtained, it was concluded that vaccination is consequential and should be continued fastidiously.
