Hastanelerde sözleşmeli hemşirelerin işgücü devri ve hizmet kalitesi

Özet

Amaç: Bu çalışma, hastanelerde gün geçtikçe sayısı artan sözleşmeli hemşirelerin işgücü devir hızını hesaplamak, Hemşirelik Hizmetlerinden Memnuniyet Ölçeğini hastalara uygulamak suretiyle, hastaneler arası karşılaştırma yaparak, işgücü devir hızı ile hizmet kalitesi arasındaki ilişkiyi incelemek amacıyla yapılmıştır. Yöntem: Çalışma için 360 hastaya anket formu ulaştırılmış olup, 13 tanesi eksik veya yanlış doldurulduğu için analiz dışı bırakılmıştır. Araştırma verilerinin toplanması için hasta bilgi formu ve hemşirelik bakımından memnuniyet ölçeği kullanılmıştır. Verilerin değerlendirilmesinde ortalama, yüzdelik yöntemleri gibi tanımlayıcı istatistikler ve Oneway Anova Testi kullanılmıştır. Bulgular: Araştırmaya katılan hastaların % 51,53’ü kadın, % 48,47’si erkek, %35,07’si 19- 39 yaş grubu, % 35,94’ü 40-59 yaş grubu, %28,99’u 60 yaş ve üstünde, % 68,41’i evli, % 16,52’ si bekar, % 13,04’ü dul, % 2, 03’ü boşanmış, % 89,2’sinin sosyal güvencesi bulunmakta, % 10, 98’inin bulunmamaktadır. Eğitim durumları incelendiğinde % 13,40’ının okur-yazar, %35,51’inin ilkokul mezunu, %16, 51’inin ortaokul mezunu, % 22,12’sinin lise mezunu, %12,46’sının üniversite mezunu olduğu saptanmıştır. Meslekleri incelendiğinde; %17,2’si emekli, %12,46’sı serbest meslek, % 10,96’sı işçi, % 59,38’i diğer meslek grubu mensubudur. Gelir düzeyi incelendiğinde % 41,84’ünün gelir durumunun kötü, % 49,55’inin orta derecede, %8, 61’inin iyi olduğu, yatış öyküsü incelendiğinde ise % 62, 83’ünün daha önce hastaneye yatış öyküsü bulunduğu, %37,17’sinin ise bulunmadığı görülmüştür. Ayrıca hastaların % 41,82’sinin kronik hastalığı bulunmakta, % 58,18’inin bulunmamakta, % 76,97’sinin yanında refakatçisi bulunmakta, % 23,03’ünün bulunmamaktadır. Bunlardan % 80,08’inin refakatçisi sürekli yanında kalmakta, % 19,92’sinin ise sadece belirli saatlerde kalmaktadır. Sonuç: Çalışma sonucunda işgücü devir hızı düşük ve yüksek olan kurumlarda hemşirelik hizmetlerinden memnuniyet oranı birbirine yakın çıkarken, işgücü devri orta derecede olan kurumlarda hemşirelik hizmetlerinden memnuniyet oranının diğerlerinden daha düşük olduğu saptanmıştır. Bu durum kurumların kaliteli hemşirelik bakımının verilmesinde işgücü devriyle farklı yöntemlerle başa çıktıklarını göstermektedir.

Abstract

Objective: The objective of this study is to calculate the turnover rate of labour contract nurses and to examine the relationship between turnover rate and service quality of nurses by applying Nursing Care Satisfaction Scale to the patients in order to make comparisons about rates of employee turnover between hospitals. Methods: For the study, 360 questionnaire forms were completed by patients and 13 forms were excluded from the analysis because of missing and incomplete data. Patient Information Form and Nursing Care Satisfaction Scale were used for data collection. Descriptive statistics such as percentages methods and One way ANOVAs test were used for evaluation of the data. Evidence: The demographic characteristics of the patients participating in the study are as follows: 51,53 % female, 48,47 % male, 35,07 % belong to 19-39 age group, 28,99 % are 60 and above, 68,41 % married, 16,52 % single, 13,04 % widowed and 2,03 % divorced. While 89,2 % of the patients have social security, 10, 98 % lack social security. When the educational status of the research group is taken into consideration, 13,40 % are literate, 35,51 % are primary school graduates, 16,51 % are secondary school graduates, 22,12 % are high school graduates and 12,46 % are university graduates. The occupational status of the patients is that 17,2 % retired, 12,46 % self-employed, 10,96 % workers and 59,38 % belong to other professions. The income level of the group is that 41,84 % low income, 49,55 % middle income and 8,61 % high income. 62,83 % of the group have hospital accommodation before, 37,17 % has entered hospital for the first time. 41,82 % of the patients have chronic illness, 58,18 % do not have. 76,97 % of the patients have companion, 23,03 % do not have. 80,08 % of the companions stay permanently, whereas 19,92 % stay for limited time. Result: As a result of the study, the nursing care satisfaction rates are found close to each other in institutions with both high and low turnover rates, whereas nursing care satisfaction rate is lower in institutions with middle turnover rate. This situation shows that in terms of nursing care quality, institutions cope with turnover rate by different methods.
