Özet

Bu çalışmanın amacı adneksial torsiyon tanısında ultrasonografinin tam etkinliğinin saptanmasıdır. Bu amaçla cerrahi kesin tanı almış 19 adneksial torsiyon olgusunun ultrason bulguları tekrar değerlendirilmiştir. Olguların %65 inde (n=13) esas olarak kistik ve %35 inde (n=6) yalnızca solid olmak üzere ultrasonografi ile adneksiel kitlerin kökenine göre üç gruba ayrıldı. Ovaryan kitleri 10 olgu Grup 1, ektraovarian kitleli 6 olgu Grup 2, adneksial kitlesi olmayan 3 olgu Grup 3 olarak sınıflandı. Hipoeksik ovaryan stroma ve periferik kistik yapılar sırasıyla %47 (n=9) ve %21 (n=4) olguda mevcuttu. Yedi olguda (%32) culdesak ta sıvı vardı. Sonuç olarak, adneksial torsiyona ait özel bir ultrason bulgusu saptanamamıştır. Torsiyona adnekse ait ultrasonografik görünüm hastanın yaşı, hormonal durumu veolayaneden olan adneksial kitlenin patolojisine göre değişmektedir.

Abstract

OBJECTIVE: The aim of this study is to determine the efficacy of ultrasound in diagnosis of adnexal torsion. STUDY DESIGN: The ultrasonographic findings of 19 surgically confirmed adnexal torsion were reviewed. RESULTS: The ultrasonography revealed an adnexal mass mainly cystic in nature in 65% (n=13) and solid only in 35% (n=6) of the patients. Cases were categorized according to the presence and origin of the adnexal mass. Group, 1,2 and 3 included 10 cases with an ovarian mass, 6 cases with an extraovarian mass and 3 cases withoud any adnexal mass respectively. Hypoechoic ovarian stroma and peripheral cystic structures were positive in 47% (n=9) and 21 % ( n=4) of all cases. Seven cases (32%) had fluid in culdesac. In group 2, 67% of the cases had hypoechoic ovarian stroma and 33% had peripheral cystic structures. CONCLUSION: We could not find any ultasonographicaljy; specific markers. The sonographic findings of adnexal torsion seem to be related to the age and hormonal status of the patient and to the pathology of the associated adnexal mass.
