Endometrial ablation with thermal balloon to treat menorrhagia and preliminary clinical results

1994 ile 1995 Aralık tarihleri arasında yapılan bu çalışmamızda, termal balon ile endometriyal ablasyon metodunu tanımak ve bu yeni metodun güvenilirliği ile etkinliğini araştırmak istedik. Tıbbi tedavi ile küretaja refrakter, menoraji şikayeti olan 19 hastaya termal balon ile endometriyal ablasyon yapıldı. İşlem sırasında, üterin perforasyon riski ile endometrium dışındaki yerlerin katmanlara termal etkinin değerlendirilebilmesi için laparoskopi ve balonun uterin kaviteye tam adapte olup olmadığının değerlendirilebilmesi için laparoskopi ve balonun uterin kaviteye tam adapte olup olmadığının değerlendirilmesi için de ultrasonografi yapıldı. Bütün hastalar 6 ay süre ile takip edildiler. 19 hastanın 12' sinde amenore, 6' sında hipomenore saptanırken, 1 hastalarda endometrial skar oluşumu histopatolojik olarak tesbit edildi. Herhangi bir komplikasyon gelişmedi.
Abstract
OBJECTIVE: The objective of this study was to recognize the ne'w endometrial ablation system with the thermal balloon and to evaluate its effectiveness and safety. STUDY DESIGN: 19 women with menorrhagia, refractory to hormonal medication and curettage underwent endometrial ablation with thermal balloon system. At the same time laparoscopy and ultrasonography were applied to evaluate the uterine cavity. All of the patients were examined on the first, third and sixth months to evaluate the amount of vaginal bleeding and an endometrial biopsy was performed to control for scar formation hystopathologiçşfly. RESULTS: 12 of 19 patients became amenorrheic and ii; hypomenorrheic and 1 had nochange. There were no perforation and thermal damage on laparoscopfc screen and latex balloon fitted the uterine cavity perfectly. Scar formation was more on the amenorrheic patients and less at the hypomenorrheics and no change was seen on the untreated one. CONCLUSION: Eendometrial ablation with thermal balloon system is an effective, safe and easy procedure for the treatment of menorrhagia and may be an acceptable alternative to hysteroscopic ablation methods.
