Neonatal morbidity in disproportionate and proportionate macrosomic infants of non-diabetic mothers

Özet

Ponderal indeksleri yüksek olmaları ile tanımlanan, disproporsiyonel makrosominin özellikle diabetik anne çocukları için karakteristik olduğu ve bu bebeklerde neonatal morbiditenin daha sık olduğu bilinmektedir. Sunduğumuz çalışmada diabetik olmayan annelerin makrosomik bebekleri bu açıdan değerlendirilmiştir. Çalışmada annelerinde gerçek veya gestasyonel diabetes mellitus olmayan ve zamanında doğan 209 makrosomik bebek değerlendirilmiştir. Doğumda resüsitasyon gereken, Apgar skorları düşük olan veya geçici takipne dışında olan bebekler makrbsbmik, bunlar arasında da ponderal indeksleri 90 persentilin üzerinde olan bebekler disproporsiyonel olarak kabul edilmiş; hematokrit, serum glukoz ve bilirubin değerleri değerlendirilmiştir. Değerlendirme sonucunda hipoglisemi, polisitemi ve hiperbilirübinemi sıklığının proporsiyonel ve disproporsiyonel makrosomik bebeklerde aynı olduğu görülmüştür. Verilerimize göre anne diabetik değilse disproporsiyonel makrosomik bebeklerde neonatal morbiditede artış görülmemektedir.

Abstract

OBJECTİVE: Disproportionatemacrosomia who have high pönderâl index ishighly specific for infantsof diabetic mothers and these infants have increased neonatal morbidity. We tested the hypotheses that macrosomic infants of non-diabeticmothers would haveincreased neonatal morbidity. STUDY DESIGN: Westudied 209 macrosomic infants of non-diabeticmothers. Mothers of infants had negativeresultson standard glucose challenge tests between 24 and 28 weeks of gestation. All infantshave a gestational age atdelivery of at least 37 weeks. Infantswho need resuscitation at birthand infants with low Apga'r scores and respiratory distress"|ttd fatal congenital malformation were also excluded from the study. Macrosomia was defined as^h weight> 90th percentile for gestational age. Proportionate versus disproportionate growth was determined by ponderal index. Proportionate macrosomia was defined as macrosomia with a normal ponderal index, and disproportionate macrosomia as macrosomia with a normal ponderalindex, and disproportionate macrosomia as macrosomia with a ponderalindex >90th percentile. Blood hematocrit.serum glucose and bilirubinconcentrations were measured in all infants. RESULTS: The incidenceof hypoglycemia, polycythemia and hyperbilirubinemia was not different between disproportionate and proportionate infants of non-diabetic mothers. CONCLUSION: Data show that disproportionate macrosomia in the infants of non-diabetic mothers is not associated with an increased likehood of neonatal complications.
