How does epidural analgesia affect labor and delivery? A controlled trial

Özet

Epidural anajezinin doğumun birinci, ikinci evresi üzerine, operatif doğum ve sezaryen üzerine etkilerini gösteren farklı yayınlar vardır. Çalışmanın amacı, epidural analjezinin doğum eylemi ve doğum üzerine etkilerini incelemekti. Termde 200 nullipar hastada, epidural anajezinin etkileri araştırıldı. Elli olguya, epidural analjezi, 50 olguya pethidine hydrochloride uygulandı. Epidural uygulanan grupta kontrol grubuna göre doğumun I. evresi 28 dakika (270.07±87.18 ve 298.36±12.52) kısa olup, istatistiki fark saptanmadı. Doğumun II. Evresinin süresi, operatif doğum ve sezaryen açısindan her iki grup arasında fark yoktu. Maternal ağrı değerlendirildiğinde, epidural uygulanan grupta, % 78, kontrol grubunda % 30, ağrı hissinde azalma saptandı. Doğru uygulanan epidural analjezinin doğum eyleminin süresini operatif doğum ve sezaryen riskini arttırmadığı ve maternal ağrı hissinde önemli ölçüde azalma sağladığı saptandı.

Abstract

Objective: The affect of epidural analgesia on the first and second stage of labor and its association with instrumental delivery and cesarean section is controversial. The purpose of this study is to assess the affect of epidural analgesia on labor and delivery. Materials and Methods: A prospective randomized study was carried out in one hundred nulliparous, term women in whom 50 underwent epidural analgesia and 50 were administered pethidine hydrochloride. Results: The first stage shortened 28 minutes in epiduralgroup (270.07±87.18 vs. 298.36+12.52) and the difference was not significant. No difference was obsef between two groups in terms of the duration of second stage, instrumental delivery and cesarean sectionVAccording to maternal pain assessment 78 % of patients in epidural group and 30 % in pethidine group had'an excellent pain relief and the difference was significant (p). Conclusion: Properly administered epidural analgesia did not influence, the duration of first and second stage of labor, instrumental delivery and cesarean section rate, but it provided an excellent analgesia.
