Delivery and outcome in nulliparous pregnant women over 35 years of age

35 yaş ve üstünde primigravid hastalarda antepartum veriler, doğum komplikasyonları and perinatal sonuçlarının değerlendirilmesi amacıyla yapılan bu çalışmada 1.1.1994 ile 31.12.1995 tarihleri arasında kliniğimizde takip edilmiş 35 yaş ve üstü nullipar 232 hastanın gebelik ve doğum kayıtları retrospektif olarak incelendi. Bu süre içinde toplam doğum sayısı 39858 idi. Bunların 1112 (%2.8) kadarını 35 yaş veüstü hastalar oluşturuyordu. Ayrıca nulipar hastaların %11.9 kadarı da >35 yaş grubundaydı. Preterm doğum 37 hastada gözlenirken (%17.4), postterm gebelik oranı %5.6 olarak bulundu$\geq$35 yaş nullipar gebelerde, <35 nullipar gebelerle karşılaştırıldığında preterm doğum oranı, neonatal yoğun bakım ünitesine çıkış ve perinatal mortalité oranı daha yüksek olarak gözlendi. Sonuçta bu çalışmamızda; ileri yaş gebeliklerinin kötü perinatal sonuçlar bakımından belirgin risk taşıdığı sonucuna vardık.

Abstract

OBJECTIVE: To assess the antenatal parameters, delivery complications and the perinatal outcome innulliparous pregnant patients of >35 years of age. STUDY DESIGN: In this retrospective study which was performed between January 1,1994 to December 31,1995, 232 nulliparous pregant women aged >35 years were reviewed on the basis of labor parameters according to the computerized data. RESULTS: In this time period total number of deliveries was fiound to be 39858 and 1112 of these were >35 years of age. (2.8%) On the other hand 11.9% of the nulliparous patients were >35 years of age. Preterm birth occurred in 37 patients (17.4%) while postterm pregnancy rate was found to be 5.6%. When nulliparous women with $\geq$35 were compared to nulliparous women below 35 years of age, preterm delivery rate, neonatal intensive care unit admission (ICU) and perinatal mortality rate were found to be significantly more frequent. CONCLUSION: We suggest that recent trends towards older childbearing poses a significant risk of poor perinatal outcome namely, preterm birth, ICU admission and perinatal mortality along with higheroperative delivery rate.
