The prognosis of fetuses with posterior urethral valve

Ultrasonografik olarak antenatal dönemde mesane çıkışı obstrüksiyonu bulgusu saptanan fetusların prognozunu belirlemek amacıyla prenatal olarak posterior valv saptanan 22 fetusun bulguları ve bizim yaklaşımımız gözden geçirilmiştir, İstanbul Tıp Fakültesi prenatal tanı merkezinde Şubat 1993 den Haziran 1998'e kadar geçen süre içerisinde 22 fetusa posterior uretral valv tanısı konulduğu belirlenmiştir. Müracaat anındaki ortalama gestasyonel yaş 20.7 hafta (12-37) bulunmuştur. Fetusların hepsinin erkek olduğu belirlenmiştir. Dört olguda ek anomalilere rastlanmış, karyotip analizi yapılan 12 olgunun hiçbirinde anormal karyotip bulunmamıştır. Sekiz olguda ilk ultrason incelemesi sırasında, dört olguda da daha sonra oligohidramniyos saptanmıştır. Ondört olguda gebelik sonlandırılmış, bir olguda mesanedeki idrar aspire edilerek boşaltılmış, başka bir olguda da fetusa vesiko-amniotik şant uygulanmıştır, iki olguda spontan rezolüsyon gözlenirken diğerlerinde ultrason bulguları değişmemiştir. Fetusların yedisi term veya terme yakın dönemlere kadar ulaşmış ve doğurtulmuş, bunlardan biri postpartum 54. günde kaybedilmiş diğerleri 2-6 yaşlarına kadar yaşamışlardır. Fetal posterior uretral valv saptanan gebelikler değerlendirilirken, amniyotik sıvı hacmi, gebelik haftası renal parankimanın görünüşü, diğer letal anomalilerin varlığı ve renal fonksiyonlar göz önünde bulundurulmalıdır.
Abstract
OBJECTIVE: To determine the factors influencing the prognosis of fetuses with bladder outlet obstruction by prenatal sonographical findings. STUDY DESIGN: We reported the prenataly diagnosed 22 fetuses that have posterior urethral valves, and documented their prognosis with our management strategies. RESULTS: We diagnosed 22 cases with posterior urethral valves during the period February 1993 to June 1998. The mean gestational age at referral was 20.7 weeks (12 to 37). All of the fetuses were male. In four cases additional abnormalities were observed. In the 12 cases karyotype analyzing performed, no chromosomal abnormality was detected. Oligohydramnios was apparent in eight cases at the first scan and, in four cases observed later. The pregnancies were terminated in 14 cases, aspiration of the urine from the bladder was performed in one case, in another case a vesico-amniotic shunt was applied to the fetus. Spontaneous resolution were observed in two, cases, sonographical findings were not changed in other cases. Seven fetuses were reached to term or near term, and were delivered. One of them died at postpartum 54th day, and the others survived until two to six years of age. CONCLUSION: In the management of the fetuses with posterior urethral valve, the evaluation should be based on the amount of amniotic fluid volume, gestational age, appearance of renal parenchyma, presence of other lethal anomalies and karyotype, and renal function.
