Retrospective analysis of clinicopathologic features in patients with uterine sarcoma

Özet

Bu makalede uterus sarkomlu 36 hastanın klinikopatolojik özelliklerinin prognoz ve sağkalım ile olan ilişkileri retrospektif olarak değerlendirilmiştir. Toplam 36 hastanın 19'u leiomyosarkom (LMS), 12'si mikst mesodermal sarkom (MMS), 3'ü endometrial stromal sarkom ve 2 tanesi de sınıflandırılmayan grupta yer almaktaydı. Ortalama hasta yaşı LMS'de 56.8, MMS'de 61.2, ESS'de 51.3 ve sınıflandırılmayan grupta 53 olarak saptandı. 25 hasta premenopozal, 11 hasta ise postmenopozal dönemdeydi. 20 hasta evre, 2 hasta evre 2, 7 hasta evre 3, 7 hasta da evre 4 hastalığa sahipti.Yirmi iki hastada sadece cerrahi, 6 hastada cerrahi ve kemoterapi, 1 hastada cerrahi ve radyoterapi, 7 hastada da cerrahi, radyoterapi ve kemoterapi uygulandı. 5 yıllık ortalama yaşam tüm hastalar için %32.4 olarak hesaplandı ve bu oran tümör tiplerine göre farklılık göstermemekteydi. Ortalama (median) yaşam süresi evre 1, 2,3 ve 4 tümörler için 65, 35, 18 ve 10 ay olarak saptandı. Çalışma sonucunda tümör evresi prognoz üzerinde etkili tek faktör olarak bulundu.

Abstract

OBJECTIVE: clinicopathologic features of 36 patients with uterine sarcoma were evaluated retrospectively. STUDY DESIGN and RESULTS: there were 19 leiomyosarcomas (LMS), 12 mixed mesodermal sarcomas (MMS), 3 endometrial stromal sarcomas (ESS) and 2 unclassified sarcomas. The mean age of the patients with LMS was 56.8 years, MMS was 61.2, ESS was 51.3 and unclassified was 53 years. 11 patients were in menopause whereas 25 patients had ongoing menstrual function. Twenty patients had stage I disease, 2 patients had stage II disease, 7 patients had stage II! disease and 7 patients had stage IV disease. Treatment modalities included surgery alone in 22 of the patients , surgery coupled with chemotherapy in 6 patients, surgery and radiotherapy in 1 patient, surgery in combination with chemotherapy and radiotherapy in the remaining 7. The overall 5 year survival of the patients was 32.4 %; in respect of tumor type it was found to be as 33.5%, 31.1%, 34,1 % and 30,3% for LMS, ESS, MMS and unclassified tumors respectively. Median survival for stage I, II, II, and IV tumors were 65, 35,18 and 10 months respectively (p<0.05). Clinicopathologic characteristics except from tumor stage did not have any effect on survival. CONCLUSION: tumor stage was the only significant factor which had significant impact on survival in uterine sarcoma.
