Clear cell carcinoma of the endometrium: The impact of surgical staging

Özet

Bu çalışmada berrak hücreli endometrium kanseri tanısı alan olgularda prognozu ve değişik tedavi şekillerinin etkinliğini ortaya koymak amaçlandı. Ocak 1992-Eylül 1999 tarihleri arasında berrak hücreli endometrium kanseri tanısı alan 20 olgu tedavi edildi. Tıbbi kayıtlar incelenerek kaba ve hastalıksız sağkalım oranları Yaşam Tablosu kullanılarak belirlendi. Hastalıksız sağkalıma etki eden prognostik faktörler Wilcoxon (Gehan) istatistiği ile saptandı. Beş yıllık kaba sağkalım oranının %62.3, beş yıllık hastalıksız sağkalım oranının %54.6 olduğu bulundu. Evre I ve II olgularda beş yıllık kaba sağkalım %100 iken bu oranın Evre III ve IV olgularda %28.5'e düştüğü belirlendi. Evre III olgularda ortanca sağkalım süresinin 17 ay olduğu saptandı. Retroperitoneal lenf nodu tutulumu (p:0.01) ve pozitif periton sitolojisi (p:0.02) en önemli prognostik faktörler olarak belirlendi. Wilcoxon (Gehan) istatistiği, Evre I ve II olgulardaki sonuçların Evre III ve IV'e göre daha iyi olduğunu ortaya koydu (p:0.02). Sonuçta bizim bulgularımız, berrak hücreli endometrium kanseri saptanan olguların tedavisinde cerrahi evrelemeyi takiben radyoterapi veya cisplatine dayalı kemoterapi ile elde edilen sonuçların kabul edilebilir olduğunu göstermektedir.

Abstract

OBJECTIVE: To establish the prognosis and effect of various treatment modalities in patients with clear cell carcinoma of the endometrium (ECCC). STUDY DESIGN: Between January 1992 and September 1999, twenty patients with ECCC were treated. All medical records were reviewed. Acturial crude and acturial disease free survival rates were calculated by using Life-Table. Wilcoxon (Gehan) statistic was used in comparing prognostic factors related with acturial disease free survival. RESULTS: Five-year acturial crude and disease free survival rates were found to be 62.3% and 54.6%, respectively. The five-year acturial crude survival was 100% for patients with Stage I and II while this rate was found to be dramatically reduced to 28.5% for patients with advanced disease (Stages III and IV). The median survival for Stage III disease was found to be 17 months. Retroperitoneal lymph node involvement was found to be the most significant prognostic factor (p:0.01) followed by positive peritoneal cytology (p:0.02) in our series. Wilcoxon (Gehan) statistic revealed that patients with low stages ECCC (Stages I and II) did significantly better than patients with advanced stages (Stages III and IV) (p:0.02). CONCLUSION: Our findings suggest that surgicopathological staging followed by either irradiation or cisplatin-based chemotherapy regarding on the extent of the disease seems to be acceptable in managing the patient with ECCC. - (Gynecol Obstet Reprod Med 2000; 6:132-136)Aq
