Ultrasound only monitoring of ovarian stimulation in IVF cycles where a long GNRH agonist protocol is used does not change the cycle outcome

Bu çalışma in vitro fertilizasyon (IVF) sikluslarmın yalnızca ultrasonografı ile monitörize edilmesinin siklus sonuclari üzerine olumsuz bir etkisinin olup olmadığını belirlemek amacıyla yapıldı. Prospektif, nonrandomize, karşılaştırmalı bir çalışmadır. Ovarian stimülasyon sırasında serum hormon değerlendirmeleri için kan aldırmayı tercih etmeyen veya IVF tedavisinin maliyetini azaltmayı isteyen 43 hastanın ilk IVF siklusları yalnızca ultrasonografi ile monitörize edildi. Aynı çalışma döneminde hem ultrasonografı hem de serum E2 ölçümleri ile monitörize edilen 34 hasta da kontrol grubunu oluşturdu. Her iki grup uzun GnRH agonist protokolü ile stimüle edildi. Ovarian stimülasyon süresi, kullanılan gonadotropin miktarı, hCG günü endometrial kalınlık ve >15 mm folikül sayısı, toplanan matür oosit sayısı, fertilizasyon oranları, elde edilen ve transfer edilen embryo sayısı, oosit pick up başına klinik gebelik oranları ve ovarian hiperstimülasyon insidansları her iki grup arasında karşilaştırıldı. Yalnızca ultrasonografi ile monitörize edilen sikluslarda fertilizasyon oranı (45 vs 56 %) ve elde edilen embryo sayısının (3.1 ± 0.4 vs 4.7 ± 0.6 ) önemli oranda daha düşük (p: 0.006 ve p: 0.013, sırasıyla), ve kullanılan gonadotropin ampul sayısının da (41.3 ± 2.6 vs 32.8 ± 1.6) önemli oranda daha fazla (p: 0.012) olmasına rağmen, oosit pick up başına klinik gebelik oranları (30 vs 26 %) ve ovarian hiperstimülasyon insidansları (7 vs 3 %) iki grup arasında farklılık göstermemektedir. Uzun GnRH agonist protokolünün kullanıldığı IVF sikluslarında, över stimülasyonunun yalnızca ultrasonografi ile monitörizasyonu geleneksel monitörizasyonun uygulandiğı sikluslar ile benzer gebelik oranları vermektedir. Fakat bu azaltılmış mönitörizasyon için uygunluk kriterleri. över rezervi ve diğer prognostik faktörler açısından iyi tanımlanmalıdır.
Abstract
OBJECTIVE: To determine if ultrasound only monitoring of in vitro fertilization cycles (IVF) has an adverse effect on the cycle outcome. STUDY DESIGN: Prospective, nonrandomized comparative study. First IVF cycles of 43 patients who refused multiple venipunctures for serum hormone determinations during ovarian stimulation or demanded for a cost reduction in IVF treatment, were monitored only by ultrasonography. 34 patients who were monitored both by ultrasonography and serum E2 determinations during the same study period served as controls. Both groups were stimulated with a long GnRH agonist protocol. Duration of ovarian stimulation, amount of gonadotropins used, endometrial thickness and the number of follicles >15 mm on the day of hCG, the number of mature oocytes retrieved, the fertilization rates, the number of embryos obtained and transferred, the clinical pregnancy rates per retrieval and the incidence of ovarian hyperstimulation in both groups were compared. RESULTS: Although the fertilization rate (45 vs 56 %) and the number of embryos obtained (3.1+0.4 vs 4.7+0.6) were significantly lower (p:0.006 and p:0.013, respectively) and the number of gonadotropin vials used (41.3±2.6 vs 32.8+1.6) was significantly higher (p:0.012) in cycles monitored by only ultrasonography, the clinical pregnancy rates per retrieval (30 vs 26%) and the incidence of ovarian hyperstimulation (7 vs 3%) does not differ significantly between the two groups. CONCLUSION: In IVF cycles where a long GnRH agonist protocol is used, ultrasound only monitoring of the ovarian stimulation gives the same pregnancy rates with the traditional monitoring. But the eligibility criteria for this reduced monitorisation should be well defined in terms of ovarian reserve and other prognostic factors.
