Abstract

Two cases qf struma ovarii clinically simulating ovarian carcinoma is presented and associated literature is reviewed in this case report. A 55 year-old perimenopausal woman with pelvic pain was admitted to the hospital. Ultrasound showed a multiloculated cystic mass in the left cranial part of uterus which measures 112 mm. in its largest diameter. Her erythrocyte sedimentation rate was 40 mm. per one hour. Her chest x-ray was normal. At elective laparotomy, a multilocular cystic mass was present possibly originating from left ovary with close adhesions to small and large bowel. During the blunt dissection of the adhesions from the bowels, the capsule of the mass was ruptured and a gelatinous mass was split away to the abdominal cavity which was thought to be malignant at tha first look The mass was detected to be torsioned around its peduncle axis for four times. Bilateral salpingo-oophorectomy and total hysterectomy was performed. No peritoneal deposits were found. Histology revealed a benign, torsioned left struma ovarii and a normal right ovary. The patient was discharged from follow-up six months later.
