Laparoscopic treatment of ectopic pregnancy: Analysis of 67consecutive cases

Özet
Bu çalışmada ektopik gebelik tedavisinde operatif laparoskopinin etkinliğini ve güvenilirliğini değerlendirmek amaçlandı. Ektopik gebelik tanısı operatif laparoskopi yapılan hastaların kayıtları retrospektif olarak incelenerek hemodinamik stabilite, tubal distorsiyonun < 5 cm. olması, pelvisin rahat ve yeterli gözlenebilmesi gibi kriterlere uyan hastalar seçildi. Haziran 1996 ve Ocak 1998 tarihleri arasında laparoskopik olarak tedavi edilen 67 ardışık ektopik gebelik vakası analiz edildi. Ondokuz vakada salpenjektomi yapılırken (28.4%), 48 vakada konservatif cerrahi uygulandı. Tam trofoblastik doku terminasyonunu değerlendirmek için laparoskopiden sonra serum (3hCG deki düşme için kısa süreli monitorizasyon yapıldı. Vakaların 97% sinde tam tedavi sağlanırken, persiste trofoblastik doku gözlenen iki vakada sistemik metotreksat tedavisi uygulandı. Sadece bir hastada şiddetli intraabdominal adezyon nedeniyle laparotomiye geçildi. Transvajinal ultrasonografi ile < 5 cm. adneksiyal kitle bulunan hemodinamik olarak stabil olan ektopik gebeliklerde iyileşme süresinde ve skar dokusunda azalma ve özellikle postoperatif hastane yatış süresindeki belirgin iyileşme nedeniyle laparoskopik cerrahi tedavi metodu olarak tercih edilmelidir.
Abstract
OBJECTIVE: To compare the effectiveness and safety of operative laparoscopy when used for the treatment of ectopic pregnancy. STUDY DESIGN: We retrospectively reviewed records of all patients with diagnostic laparoscopy documenting the presence of an ectopic pregnancy. Patients who met the following criteria were selected: hemodynamic stability, tubal distortion no greater than 5 cm and adequate visualization of the pelvis. RESULTS: A total of 67 consecutive patients who were treated laparoscopically between June1996 and January 1998 were analysed. Conservative surgery was performed in 48 cases (71.6%), whereas 19 cases were treated by salpingectomy (28.4%). A short term follow up was carried out monitoring postsurgery serum phCG decrements to evaluate the complete trophoblastic tissue removal. Complete resolution of ectopic pregnancy was reached in 97% of the cases, while two patients revealed persistent trophoblastic tissue and received systemic administration of methotrexate. Only one laparotomy had to be performed due to severe intraabdominal adhesions. CONCLUSION: Laparoscopy should be considered as the surgical treatment of choice for ectopic pregnancy in hemodynamically stable patients with an adnexial mass smaller than 5 cm by transvaginal sonography, because of the considerable reduction in recuperation time, incisional scarring and especially postoperative stays.
