Comparative effects of two different oral formulations of hormone replacement therapy on insulin-like growth factor-1 (IGF-1) and insulin-like growth factor binding protein-3 (IGFBP-3) levels

Özet

Bu çalışmada, 82 sağlıklı postmenopozal olguya iki farklı formda oral yolla uygulanan hormon replasman tedavisi verilerek dolaşımdaki IGF-1 konsantrasyonları üzerindeki etkisi araştırıldı. Bir gruba sadece 2.5 mg/gün tibolone, diğer gruba ise 0.625 mg konjuge östrojen+2.5mg/gün medroksiprogesteron asetat üç ay süre ile uygulandı. Serum IGF-1 değerleri nde tibolone grubunda istatistiksel olarak anlamlı olmayan (p=0.08) bir düşüş gözlenirken, östrojen ve progesteron uygulanan grupta IGF-1 değerlerinde istatistiksel olarak anlamlı oranda (p<0.001) düşüş sa ptandı.Her iki gruptada serum IGFBP-3 değerlerinde anlamlı bir değişiklik gözlenmedi. IGF-1'in oral yolla uygulanan hormon replasman tedavilerii ile kronik olarak baslılanması kas kitlesi ve diğer IGF bağımlı dokularda uzun dönemde olumsuz etkilere yol açabilir. Tibolone kullananlarda anlamlı olmamakla birlikte her iki hormon replasman tedaviside IGF-1 değerlerinde azalmaya neden olmaktadır.

Abstract

OBJECTIVE: In this study, we tried to determine whether circulating IGF-1 concentrations could be restored by oral administration of two distinct common formulations of postmenopausal hormone replacement therapy. STUDY DESIGN: 82 healthy spontaneous postmenopausal women were randomly divided into two groups to receive either tibolone 2.5 mg daily or conjugated estrogens 0.625 mg plus medroxyprogesteron acetate 2.5 mg daily for three months. Serum IGF-1 and IGFBP-3 levels were evaluated before and at the end of the study. Unpaired and paired t-tests were used to compare the data. RESULTS: serum IGF-1 levels in women in tibolone group showed a decrease which was not significant (p=0.08) while estrogen and progesterone group demonstrated a significant decrease in IGF-1 levels (p<0.001) using paired t-test. We did not observe any significant changes in serum levels of IGFBP-3 with either kind of drugs. CONCLUSION: Chronic suppression of circulating IGF-1 that occurs with the oral route may have long-term negative effects on muscle mass and other IGF responsive tissues. Both routes of hormone replacement therapy cause a decrease in IGF-1 levels which is not significant in tibolone users.
