Early diagnosis of genital tuberculosis

Özet
Bakteriyolojik olarak pozitif, tanısı yeni konulmuş ve henüz tedaviye başlanmamış olan erken dönemdeki 28 pulmoner tüberkülozlu kadın çalışmaya dahil edilmiştir. Kadınlardan ayrıntılı jinekolojik öykü alınıp, fizik muayeneleri yapıldıktan sonra histerosalpingografik inceleme ve endometrial küretaj materyalinin bakteriyolojik-histopatolojik incelemesi yapıldı. Değerlendirmeler sonucunda, kadınların hiçbirinde genital tüberküloz bulgusuna rastlanmadı. Sonuç olarak; bulgularımızı, literatür bilgileri ile birlikte değerlendirdiğimizde, erken pulmoner tüberküloz döneminde tanısı konulduktan sonra uygun tedavisi gerçekleştirilen akciğer tüberkülozlu kadınlarda, genital tüberküloz ve buna sekonder olarak gelişen tüberküloza bağlı infertilite olguları azaltılabilecektir.
Abstract
OBJECTIVE: This study was planned to determine the genital tract involvement at the early stage of active pulmonary tuberculosis. STUDY DESIGN: Twenty-eight cases of bacteriologically positive, newly diagnosed, uncomplicated and untreated pulmonary tuberculosis were included in this study. Patients were submitted to a detailed gynecological history, physical examination, hysterosalphingography (HSG) and endometrial curettage for bacteriological and histological examination. RESULTS: The findings of pelvic examination, HSG and endometrial samplings did not determine any genital tuberculosis at the very early stage of pulmonary tuberculosis. CONCLUSION: Genital tuberculosis may be rarely seen at the early stage of pulmonary tuberculosis, unless the bacilli is resistant and patients come to clinics at later stages of the disease. It is suggested that, if pulmonary tuberculosis cases are diagnosed and treated early before the genital tract involvement, tuberculous infertility cases will decrease.
