Heterotopic triplet pregnancy: Report of a case

Özet
intrauterin çoğul gebelik olgularında ektopik gebelik birlikteliği oldukça nadirdir. Bununla birlikte insidans, pelvik inflamatuar hastalık, ovulasyon indüksiyonunda kullanılan ilaçlar gibi birçok risk faktörleri nedeniyle giderek artmaktadır. Heterotopik gebelik tanısı gerektiği kadar erken dönemde konamamaktadır ve ciddi yan etkiler söz konusudur. Bu yayında 75 IU LH ve FSH ile ovulasyon indüksiyonu yapıldıktan sonra heterotopik gebelik gelişen bir olgu sunuyoruz. Olgu 3 kürden sonra tedaviyi terk etmişti. 3 ay sonra minimal vajinal kanama ile başvurdu. Ultrasonda 5 haftalık heterotopik triplet- intrauterin ikiz ve ekstrauterin tek-gebelik saptandı, izlemde fetal kardiak aktiviteler kayboldu. Gebeliğin sonlandırılmasında intrauterin için D&C, ekstrauterin için tek doz Methotrexate kullanıldı. Bu olgu, risk faktörleri taşıyan olgularda ektopik gebelik birlikteliğinden şüphelenilmesi gerektiğini vurgulamaktadır. Erken sonografi heterotopik gebelik tanısı için esastır ve intrauterin çoğul gebelik varlığı bile ektopik gebelik riskini dışlamaz. Adneksiyel alan her vakada ve özellikle risk faktörü taşıyanlarda dikkatlice taranmalıdır.
Abstract
OBJECTIVE: Coexistence of ectopic pregnancy in patients with intrauterine multiple pregnancy is very rare. However an inclination of incidence arises due to a number of risk factors, most notably, pelvic inflammatory disease and the use of drugs to induce ovulation. The diagnosis of heterotopic pregnancy is frequently done not as earlier as it should be, and it has serious repercussions. CASE REPORT: We present such a case who was given ovulation induction with 75 ILJ leutinizing hormone and follicle-stimulating hormone. After three courses she had abondened the therapy. Three months later, the patient admitted with minimal vaginal bleeding. Ultrasound revealed heterotopic triplet pregnancy -intrauterine twin and extrauterine single- with a gestational age of 5 weeks. In the follow up, the cardiac activities of the fetuses were no longer observed. For termination of the pregnancy, D&C were performed for the intrauterine fetuses and a single dose of methotrexate was used for the ectopic one. CONCLUSION: This case illustrates the need to suspect the coexistence of an ectopic pregnancy in any women who has the risk factors. Early sonography is essential for the diagnosis of heterotopic pregnancy and even intrauterine multiple pregnancy does not exclude the risk of ectopic pregnancy. The adnexa should be scanned in every case; especially those with risk factors.
