Malignant melanoma of the vulva; presentation of two cases

Özet
Vulvar malign melanom tanısı konulan ve tedavi edilen iki olguyu sunmayı amaçladık. ynecology Obstetrics & Reproductive Medicine 2000; 6:155-157 157 Ilk hasta 42 yaşında idi. Fizik muayenesinde sol labium minorda lokalize 2 cm çapında lezyonu vardı. Alınmış olan biopsi materyalindeki patoloji sonucunun fuziform hücreli melanositik tümör gelmesi üzerine hastaya hemivulvektomi uygulandı. Hasta postoperatif dönemde üç kür sistemik kemoterapi aldı. Postoperatif dönemde cerrahi sınırlarda tümör saptanmamasına ve takip esnasında sorun olmamasına rağmen, ameliyattan 23 ay sonra çekilen kranial BT 'de sol frontal lob ve sağ gözde malign melanom metastazları izlendi. Bunun üzerine hastaya radyoterapi uygulanmasına karar verildi. Ancak uygulanan tedaviye rağmen hasta tanı konduktan toplam 26 ay sonra kaybedildi. Diğer hastamız 60 yaşındaydı. Fizik muayenesinde her iki labium minor ve majörün anterior kısmında geniş bir lezyonu vardı. Biopsi sonrası üç çizgi radikal vulvektomi ve bilateral inguinal lenfadenektomi yapıldı. Cerrahi sınırlarda ve lenf ganglionlarında tümöre rastlanmadı. 10. ayın sonunda sol inguinal bölgede lenf ganglionları palpe edildi. Hasanın fizik muayenesinde ve bilgisayarlı tomografi yardımı ile yapılan radyolojik görüntelemesinde ek metastatik odağa rastlanmadı. Bu nedenle inguinal bölgedeki kitle lokal anestezi altında eksize edildi. Patolojik cevap malign melanom metastazı olarak rapor edildi. Yapılan bu ikinci cerrahi girişimden bir ay sonra sol akciğer lobunda metastazla uyumlu kitle belirdi. Bu metastazdan dolayı solunum sıkıntısı gelişen hasta ilk ameliyatından 13 ay sonra hayatını kaybetti. Vulva malign melanomların tedavisinde radikal cerrahi girişimin yeri tartışmalıdır. Her iki olgumuzda da cerrahi sınırların tümör negatif olmasına rağmen kısa bir süre içinde beyin ve akciğer metastazları gelişti. Nadir görülen vulvanın malign melanomlarının radikal cerrahisi uzun dönem pronoz üzerine etkili olmamaktadır.
Abstract
Our purpose was to describe two vulvar malignant melanoma cases, which were treated with surgery, radiotherapy and chemotherapy. The first patient was a 42-year old woman with a lesion of two cm. in diameter, localized on the left labium minus confirmed by physical examination. Left hemivulvectomy with negative surgical margins was performed after biopsy, reporting fusiform cell melanocytic tumor. She received three doses of systemic chemotherapy postoperatively. In spite of negative surgical margins, metastases were detected in the left frontal lobe and right eye on computed tomography 23 months postoperatively; thus she received cranial radiotherapy and died 26 months after the diagnosis of disease. The second case was a 60-year old woman. On her gynecologic examination there were lesions on her upper parts of both labium majora and minora. After confirming the diagnosis of malignant melanoma by biopsy, "three line" radical vulvectomy and bilateral inguinal lymphadenectomy was performed. The surgical margins were negative and no tumor was found on inguinal lymph nodes. After ten months, palpable lymph nodes were found in the left inguinal region. The physical examination and tomographies were unable to detect another metastatic site. Hence this mass was removed surgically. After a month from the second operation a metastatic lesion was detected in the left lung. The patient developed a respiratory insufficiency due to this metastasis and died after 13 months from the first operation. The role of radical surgical management in the treatment of vulvar malignant melanoma is controversial. Although surgical margins were negative in both of our patients, brain and lung metastases were detected one or two years after primary surgery.
