The effect of zinc deficiency and unexplained elevated maternal serum alfa-fetoprotein levels on pregnancy complications

Gebelik komplikasyonları ve kötü gebelik sonuçlarının erken tanısında ikinci trimester maternal serum alfa-fetoprotein (MSAFP) ve çinko seviyelerinin tanısal etkinliğinin, her iki incelemenin birlikte değerlendirilmesiyle arttırılması amacıyla 16-18. gebelik haftasında 160 gebede serum AFP ve çinko seviyeleri ölçüldü. Çinko analizi atomik absorbsiyon spektrofotometri tekniğiyle yapıldı AFP analizi radyoimmünoassay tekniğiyle yapıldı. Gebelik sonucu bilinen 127 hastada MSAFP ve çinko yoğunluklarının korelasyon göstermediği tespit edilmiştir (r=0.27). Serum çinko eksikliğinin ( 2 MoM) durumunda ise; oligohidramnios, SGA, PIH riskinde artış olduğu ve gebelik süresinin kısaldığı bulunmuştur (p< 0.05). iki testin birlikte bozuk olduğunda sadece SGA oranının arttığı görülmüştür (p< 0.05). Gebelik komplikasyonları ve kötü gebelik sonuçlarının erken tespitinde MSAFP'e ek olarak çinko seviyesinin değerlendirilmesi tanısal etkinliği arttırmamıştır. Buna karşın, üçlü tarama testinin bir komponenti olarak yaygın kullanılan MSAFP ölçümünden gebelik komplikasyonları ve kötü gebelik sonuçlarının erken tanısında da yararlanılması gözardı edilmemelidir.
Abstract
OBJECTIVE: To increase the diagnostic efficiency of 2<sup>nd</sup>trimester maternal serum alfa-fetoprotein (MSAFP) and zinc assessment tests for pregnancy complications and adverse pregnancy outcome, when both tests were used together. STUDY DESIGN: Maternal serum zinc and MSAFP levels , measured between 16 to 18 weeks of pregnancy, were correlated to pregnancy complications and adverse pregnancy outcome in a prospective study design. RESULTS: Serum levels of AFP and zinc were not correlated with each other (r= 0.27). Serum zinc deficiency (a 10 <sup>th</sup>percentile serum zinc level of <45 µgr./dl) was neither associated with adverse pregnancy outcome nor increased the risk of pregnancy complications. But, unexplained high levels of MSAFP (> 2 MoM) were found to be significantly associated with an increased risk of oligohydramnios, small for gestational age (SGA) fetus and pregnancy induced hypertension complications and short duration of pregnancy (p< 0.05). When both tests were abnormal, only the risk of SGA fetus were found to be increased (p< 0.05). CONCLUSION: The combination of serum zinc assessment to MSAFP test did not increase the diagnostic efficiency for the pregnancy complications and adverse pregnancy outcome . But, the capability of 2nd trimester MSAFP assessment, as a part of triple test screening, for the early diagnosis of pregnancy complications and adverse pregnancy outcome should always be kept in mind.
