Evaluation of myometrial invasion by transvaginal ultrasonography in endometrial carcinoma

Özet
Bu çalışmada endometriyum kanserinde lenf nodu tutulumu ile doğrudan ilişkili olan, tedavinin planlanmasında ve prognozu belirlemede büyük önem taşıyan myometrial invazyonun derinliğinin saptanmasında transvajinal sonografinin etkinliğini araştırmak amaçlandı. Endometriyum kanseri tanısayla opere edilen 46 hasta çalışmaya alındı. Preoperatif dönemde hastalara transvajinal ultrasonografi yapılarak myometriyal invazyonun derinliği sagittal planda alınan görüntülerde değerlendirildi. Histerektomi materyallerinin histopatolojik değerlendirilmesi ile myometriyal invazyon derinliği tespit edildi. Preoperatif ultrasonografi sonuçları ile postoperatif histopatoloji sonuçları karşılaştırılarak myometriyal invazyonu belirlemede transvaginal ulrasonografinin doğruluğu, derin myometriyal invazyonu belirlemede ise doğruluğu, sensitivite ve spesifisitesi hesaplandı. Hastaların 36'sında ulrasonografi ile histopatoloji sonuçları aynı idi. Myometriyal invazyonu belirlemede ulrasonografinin doğruluğu %78. 2 olarak hesaplandı. Derin myometrial invazyonu belirlemede doğruluk oranı %82. 6, sensitivite %82. 6, s pesifisite %91. 3 olarak bulundu. Endometriyum kanserinde, preoperatif dönemde, myometrial invazyon derinliğinin belirlenmesinde transvaginal ulrasonografi yararlı bir yöntemdir
Abstract
OBJECTIVE: To study the effectivity of transvaginal ultrasonography in the determination of the depth of myometrial invasion which is directly related to the lymph node infiltration and has direct role in the therapy and prognosis in endometrial carcinoma. STUDY DESIGN: Fourty six patients who were operated due to endometrial carcinoma were included in the study. The depth of myometrial invasion was evaluated on the sagittal sonograms obtained transvaginally in preoperative period. The histerectomy materials were histopathologically evaluated for detecting the depth of myometrial invasion. With the comparison of the preoperative ultrasonography results and postoperative histopothological results, the accuracy, sensitivity, and specificity of ultrasonography in the determination of myometrial invasion were calculated. RESULTS: In 36 patients, the transvaginal ultrasonography and the histopathological results were the same. The accuracy of sonography was 78. 2% in the detection of the depth of myometrial invasion. In the detection of deep myometrial invasion, the accuracy, sensitivity, and specificity were 82. 6%, 82. 6%, and 91. 3%, respectively. CONCLUSION: In preoperative period, transvaginal ultrasonography is a useful method to evaluate the depth of myometrial invasion in endometrial carcinoma. (Gynecol Obstet Reprod Med 2001; 7:146-148)
