Conservative surgery for ovarian cancer in women of childbearing age

Özet
Bu makalede genç yaşta olan ve çocuk doğurma isteği devam eden hastalardaki konservatif cerrahinin yeri araştırılmıştır. Ankara Üniversitesi Tıp Fakültesi'nde öpere olmuş 5 hasta ile kırsal bir hastanede basit over kisti nedeni ile ilk olarak öpere edilen ve daha sonra rekürens ile kliniğimize başvuran toplam 6 hastanın kayıtları t değerlendirilmiştir. Bütün hastalara unilateral öoferektomi, gygun cerrahi evreleme ve karşı over ile uteruşun korunduğu ve karşı överden biyopsi alınması ile kârakterize olan konservatif cerrahi uygulanmıştır. Hastaların histopatolojik değerlendirilmesinde 2 epitelyal over kanseri, 3 disgerminoma ve 1 immatür teratoma bulunmuştur. İzlem süresince hiçbir hastada relaps gözlenmezken, ortalama hastalıksız geçen süre epitelyal over kanserli hastalar için sırasıyla 55 ve 111 ay, germ hücreli tümörler için 16, 41 ve 111 ay Ve immatür teratoma için 53 ay olarak bulunmuştur. Sonuç olarak konservatif cerrahi genç yaştaki çocuk doğurma arzusu olan hastalar için radikal cerrahiye karşı etkin bir alternatif tedavi seçeneği olmaktadır.
Abstract
OBJECTIVE: To investigate the effectiveness of conservative surgery in the treatment of ovarian cancer in women desiring fertility to be preserved. STUDY DESIGN: Hospital records of 6 patients diagnosed as primary ovarian carcinoma who were operated in the Obstetrics and Gynecology Clinics of Ankara University School of Medicine between 1991 and 1997 were reviewed. There were three dysgerminomas (stages IA, IB.IC), one serous papillary cyst adeno carcinomas (stage IC), one undifferentiated adenocarcinoma and one immature teratoma (stage IA). The age distribution of the patients were 15-30 years. Conservative treatment included conservative surgery with preservation of one ovary and the uterus with biopsy of the contralateral ovary and adequate staging. RESULTS: All patients are alive without evidence of disease during the paper has been prepared. The disease-free survival was 55 and 113 months for stage IA and IC epithelial ovarian tumors respectively, 53 months for immature teratoma. Of dysgerminomas it was found to be 111 months for stage IA and 41 months for stage IB diseases respectively. The last one had referred to our clinic with relapse after 16 months from a simple ovarian cystectomy which was not known to be a dysgerminoma at the initial operation. The disease-free survival of this patient was 16 months from the debulking surgery. CONCLUSION: For women of young age who desires fertility to be preserved, conservative surgery is an effective alternative to radical surgery in curing the disease with malignant ovarian germ cell tumors or early stage epithelial ovarian carcinoma.
