Silver nitrate treatment of bartholin gland abscesses and cysts

Özet
Bartholin absesi veya kisti tanısı alan hastalarda gümüş nitrat (AGNO3) tedavisinin etkinliğini ve sonuçlarını değerlendirmek. Bu prospek tif randomize çalışmaya Bartholin absesi veya kisti tanısı alan 146 hasta dahil edildi. Bu hastalara lokal anestezi altında, poliklinik şartlarında kist veya abse lojunda gümüş nitrat çubukları (0.5cm büyüklüğünde) 72 saat süreyle bırakıldı.146 hastadan 1 1 9'unda kist (1.Grup) 27'inde abse (2.Grup) formasyonu tek veya iki taraflı idi.Grup-1 de 11 olgu grup-2 de 5 olguda iki taraflı kist veya abse formasyonu mevcut idi.Bütün diğer olgular ise tek taraflıydı.Grup-1 de semptomatik Bartholin kisti olan 3 hamile kadında aynı yöntemle tedavi edildi. Her iki gruptaki hastaların yaşları ve Bartholin absesi ve kistlerinin boyutları arasında anlamlı bir fark mevcut değildi. Tüm hastalarda 2 veya 3 hafta içinde tam bir iyileşme gözlendi.Toplam nüks oranı 6 ay içinde % 4.7 olarak belirlendi. Nüks olguların tamamı başka bir komplikasyon göstermeksizin aynı yöntemle tedavi edildiler.Grup-1 18 hasta grup-2 de ise 9 hasta daha önce farklı yöntemlerle tedavi edilmişlerdi. Bu hastalar eski nüks olgular olarak kabul edildiler. Toplam 5 hastada vulvada kimyasal çok ciddi olmayan yanıklar tespit edildi.27 aylık bir izlemde 1.grupta 5 (%4.2) 2.grupta ise 2(%7.4) yeni nüks olgusu tespit edildi. Bu çalışmanın sonucunda Bartholin abse ve kist olgularında bu tedavi yönteminin ilk seçenek olması gerektiği belirgin bir şekilde ortaya kondu.Bu yöntem diğer konvansiyonel yöntemlere göre daha kısa operasyon ve iyileşme süresi gibi avantajlar sağlar iken aynı zamanda düşük başarısızlık ve daha az komplikasyon oranlarına sahiptir
Abstract
OBJECTIVE: To evaluate the efficiency and results of silver nitrate (AgNO3) treatment modality in patients with Bartholin abscesses and cysts. STUDY DESIGN: This prospective randomized study reports results of 146 patients with Bartholin abscesses or cysts who were treated by silver nitrate (AgNO3) stick insertion(0.5cm size) into the cyst or abscess cavity for 72 hours under the local anesthesia in outpatient clinic. Out of 146 patients 119 had cysts (Group-1) and 27 had abscesses (Group-2) unilaterally or bileterally. In group-1 11 cases and in group-2 5 cases had bileteral Bartholin cysts or abscesses. All other cases were unilateral. In group-1 there was 3 pregnant women with symptomatic unilateral Bartholin cysts. These patients were also treated with same method. Two groups were similar with regard to age and the size of the Bartholin's cysts, abscesses. RESULTS: All patients showed complete healing within 2 or 3 weeks time. Overall the recurrence rate was 4.7% in 6 months time. The recurrence cases were treated again by silver nitrate stick insertion with no further complication. In group-1 18 cases and in group-2 9 cases were treated before by different treatment modalities such as aspiration, partial cyst excision and marsupialization, we accepted these patients as old recurrence cases. Chemical burning in the vulva which were not so serious was observed in 5 patients totally. There was 5 new recurrence cases in group-1(4.2%) and 2(7.4%) in group-2 in a follow up period of 27 months. CONCLUSION: At the end of the study it was very evident that this treatment modality should be the first choice in Bartholin's gland abscesses or cysts because of several advantages such as shorter operating and healing time over conventional methods with a very low failure rate and with fewer complication.
