Hysteroscopic treatment of uterine septum: Reproductive outcome

Özet
Bu çalışmada histeroskopik septum rezeksiyonu yapılan hastaların gebelik oranlarının incelenmesi amaçlanmıştır. Uterin septum nedeni ile başvuran ve histeroskopik septum rezeksiyonu yapılmış olan 33 hasta, retrospektif olarak gebelik oranları açısından değerlendirilmiştir. Primer infertilite nedeniyle opere olan 10 hastanın 4'ünde (%40), gebelik komplikasyonları olan 21 hastanın 17'sinde (%80.9), dismenoresi olan 2 hastanın 1'inde (%50) histeroskopik septum rezeksiyonunu takiben gebelik elde edildi. Histeroskopik septum rezeksiyonu, uterin septumun tedavisinde kullanılabilecek güvenilir ve uygulaması basit bir işlemdir. İşlemi takiben yüksek iyi gebelik oranları elde edilebilmektedir.
Abstract
OBJECTIVE: We aimed to evaluate the effect of hysteroscopic septum resection on reproductive outcome in infertile women. STUDY DESIGN: We retrospectively evaluated the hospital records of 33 patients between 1997 and 1999. Diagnosis of uterine septum was confirmed by hysterosalphingography and ultrasonography in all of the patients. All patients undergone diagnostic laparoscopy to confirm a normal fundal configuration and after this procedure hysteroscopic resection of uterine septum was performed. RESULTS: The mean age of the patients was 29 years old (range 18-39). Ten patients in which infertility was the main presenting symptom, 4 conceived after the procedure (40%), 3 of them had an uneventful pregnancy whereas the other miscarriaged in the first trimester. Of the 21 patients who applied for various pregnancy associated complications 17 patients conceived (80.9%). 2 first and 2 second trimester abortions were detected. 13 patients proceeded until term and gave birth to healthy infants. One of the 2 patients (50%) with the symptom of dysmenorrhea conceived after the procedure. CONCLUSION: Hysteroscopic uterine septum resection is a safe, simple and reproducible technique in any type of uterine septum. It attains fairly good obstetric outcomes.
