Laparoscopic findings in cases with the diagnosis of chronic pelvic pain

Bu çalışmada, kronik pelvik ağrı tanısıyla opere olan 67 hastanın laparoskopik bulguların değerlendirilmesi amaçlandı. Altı aydan daha uzun süre ile pelvik ağrısı olan üreme çağındaki 67 hasta çalışmaya dahil edildi. Hastaların hepsinde pelvik muayane ve laboratuvar bulguları normaldi. Hastalarda kronik pelvik ağrıya neden olabilecek patolojiyi belirleye bilmek için diagnostik laparoskopi uygulandı. Diagnostik laparoskopiye alınan 67 hastanın 21'inde hiçbir patoloji tesbit edilmedi. Hastaların 14'ünde pelvik yapışıklık, 8'inde artmış pelvik damarlanma ve 9'unda polikistik görünümde over varlığı tesbit edildi. Hastaların 12'sinde endometriotik lezyon tesbit edilirken 3'ünde subseröz myom vardı. Kronik pelvik ağrı üreme çağı kadınlarında sıklıkla görülmesine rağmen patogenezi net olarak anlaşılamamıştır. Hem hasta hem de hekim için sorun olan kronik pelvik ağrının tanısında ve tedavisinde laparoskopi etkin bir yöntemdir.
Abstract
OBJECTIVE: The aim of this study is to evaluate our laparoscopic findings in 67 patients with the diagnosis of chronic pelvic pain. STUDY DESIGN: We examined 67 reproductive age group of women who admitted to our hospital for chronic pelvic pain of at least six months duration and had been given various kinds of medical treatment. Pelvic examinations and laboratory investigations were normal in all cases. In order to elucidate any pathology, diagnostic laparoscopy was performed. RESULTS: In 67 patients who underwent diagnostic laparoscopy, 21 of them had no pathology. Pelvic adhesion were detected in 14 of them. Eight patients had increased pelvic vascularity and polycystic appearing ovaries were seen in nine patients. 12 patients had endometriotic lesions and 3 patients had subserous myomas. CONCLUSION: Chronic pelvic pain is still a problem in gynecology and is also a source of distress for both the patient and the physician. Chronic pelvic pain is mostly seen in women in reproductive age group and its pathogenesis is poorly understood. It appears that laparoscopy is an effective way to diagnose and manage the chronic pelvic pain. (Gynecol Obstet Reprod Med 2001; 7:135-136)
