Retrospective analysis of sixteen cases of primary tuba adenocarcinoma

Özet
Bu çalışmada primer tuba adenokarsinomu vakalarının incelenmesi amaçlandı. Retrospektif olarak Şubat 1992- Aralık 1999 tarihleri arasında kliniğimizde tedavi edilen onaltı primer tuba karsinomu olgusu, yaş, parite, semptom, evre, hastalıksız yaşam süresi ve toplam yaşam süresi özellikleri açısından incelendi. Tanıda olguların yaş ortalaması 57.4/yıl ( yaş aralığı: 47-76); %25'i 50 yaş altı olarak saptandı. Olguların 11'de (%68) parite 3 ve üstü olduğu görüldü. En sık karşılaşılan semptom vajinal kanama ve pelvik ağrı olarak saptandı( %38-%38). Olguların evrelere göre dağılımı incelendiğinde, % 58.3'nün evre III ve IV olduğu saptandı. Tüm olgular değerlendirildiğinde toplam yaşam süresi 45.9 ay, hastalıksız yaşam süresi 42.5 ay olarak saptandı. Primer tuba karsinomuna jinekolojik maligniteler içinde nadir rastlanmaktadır. Yaşam süresi üzerine en güçlü etki, modifiye FİGO sistemine göre tesbit edilen evre olmaktadır. Çalışmamızda olguların %58.3'ü ileri evrede( III-IV) saptandı ve %60'da kötü diferansiye tümör bulunmaktaydı. Tuba karsinomu cerrahi ve platin based kemoterapi ile tedavi edilmelidir
Abstract
OBJECTIVE: Analysis of fallopian tube carcinoma cases. STUDY DESIGN: In our Oncology Clinic, 16 fallopian tube carcinoma cases were treated between February 1992-December 1999 and these cases were analyzed retrospectively for their age, parity, symptoms, stage, disease free survival and survival. RESULTS: After the diagnosis, it was found that the mean age was 57.4 years (range of the ages 47-76) and 25% under age 50. In 11 of the cases (68%) parity was 3 and above. The most frequently encountered symptoms were vaginal bleeding and pelvic pain (38%-38%). Analysis of distribution of cases to stages showed that 58.3% were in stage III-IV. Considering all of the cases, it was found out that survival was 45.9 months and disease free survival was 42.5 months. CONCLUSION: Primary fallopian tube carcinoma is a rare gynecologic malignancy. Staging according to the modified FIGO system had the strongest influence on survival. In our study, 58.3% of total patients had advanced stage diseases (III-IV) and 60% had poorly differentiated tumors. Fallopian tube carcinoma should be treated by surgery and platinum based chemotherapy. (Gynecol Obstet Reprod Med 2001; 7:155-158)
