Effectiveness of ovulation induction and intrauterine insemination in heterogenous subfertile couples

Özet
Bu çalışma ile subfertil populasyonda ovulasyon indüksiyonu ve intrauterin inseminasyon uygulamasının etkinliğinin değerlendirilmesi amaçlandı. Retrospektif olarak ovulasyon indüksiyonu[klomifen sitrat(8), human menopozal gonadotropin(43),pürifiye üriner folikül stümülan hormon(12)] ve intrauterin inseminasyon uygulanan 37 hasta üzerinde 63 si klüs değerlendirildi. Olgular infertilite etyolojilerine göre açıklanamayan 11, hafif male faktör 15, hafif-orta endometriosis 2, hafif tubaovaryan hastalık 5 ve polikistik over 2 olarak ayrıldı. Iki olguda hem unilateral tubal oklüzyon hem de orta derece d e endometriosis saptandı. Takiben kadın yaşı( =30), infertilite süresi( =6yıl), tipi(primer, sekonder) ve infertilite nedenlerine göre olgular değerlendirildi. Gebelik oranları içinde en yüksek gebelik %11.4 ile 30 yaş altındaki hastalarda elde edildi. Fakat çalışma grubunda otuz yaş üzerinde hasta bulunmamaktaydı. Infertilite süresinde gruplar arasında sonuçlar benzer olarak bulundu( %6,2- %6,3). Primer infertil çiftlerde sekonder infertil çiftlere göre daha düşük gebelik elde edildi( %5,6-%10,0). Etyolojiye göre endometriosis ve polikistik over bulunan grupta gebelik gözlenmedi. Çalışmamızda ovulasyon indüksiyonu ve IUI tedavisinde, primer endikasyon, siklüs başına %10 gebelik oranıyla açıklanamayan infertilite olarak bulundu. En başarılı sonuç, yaş<30,sekonder infertil ve etyolojide açıklanamayan infertilite bulunan grupta alındı.
Abstract
OBJECTIVE: To assess the effectiveness of ovulation induction and intrauterine insemination (IUI) combination therapy in subfertile population. STUDY DESIGN: A total of 63 IUI cycles in which clomiphene citrate(8), human menopausal gonadotrophin (hMG) (43), urinary purify FSH(12), had been used for ovarian stimulation were analysed retrospectively to assess prognostic factors regarding treatment outcome. Thirty seven couples had undergone a basic infertility evaluation and divided into the categories of infertility etiology as unexplained(11), mild male factor(15), minimal to mild endometriosis(2), mild tubaovarian disorders(5), and ovulatory disorders(2). Two cases had both unilateral tubal occlusion and moderate endometriosis.The variables selected for the initial analysis were female age(<30,>=30), duration of infertility(<6year,>=6year), type(primary,secondary), diagnosis of infertility and sperm quality. RESULTS: The highest pregnancy rate (11.4%) was achieved in women under 30 year old, whereas no patient over 30 years conceived during the treatment cycle. Pregnancy rate was similar between short and long term infertility subgroups( 6,2%versus 6,3%). But, the primary infertile women had lower pregnancy rate than those in secondary infertile women(5.6% versus 10%). No pregnancies were observed in the endometriosis and anovulatory subgroups. CONCLUSION: The primary indication for ovulation induction with IUI in this study was unexplained infertility with a 10% conception rate per cycle. Favorable patients' characteristics for treatment success are age <30 years, secondary infertility and unexplained infertility (Gynecol Obstet Reprod Med 2001; 7:99-102)
