Intrauterine growth restriction and measures of lipid metabolism: Association between maternal and fetal metabolic status

Özet

Normal gebeliklerle intrauterin gelişme kısıtlılığı(IUGR) tanısı alan gebeliklerde anne kanı ve umbilikal kordon kanında karbonhidrat ve lipid metabolizmasındaki değişiklikler araştırıldı. Çalışmaya 33-41. gebelik haftaları arasında, sistemik bir hastalık öyküsü bulunmayan, 23 normal, 13 IUGR tanılı toplam 36 gebe katıldı. Her iki grupta, doğum eyleminin başlamasından sonra alınan maternal venöz plazma örneklerinde ve doğumdan hemen sonra elde edilen umbilikal kord venöz ve arteriyel kan plazma örneklerinde glukoz, total kolesterol, trigliserid, HDL, LDL, VLDL-kolesterol ve serbest yağ asitlerinin düzeyleri ölçüldü. IUGR grubu; maternal kan örneklerinde glukoz düzeyleri kontrol grubu ile karşılaştırıldığında istatistiksel olarak anlamlı bir düşüş gösterirken; total kolesterol, trigliserid, LDL ve VLDL-kol. düzeyleri anlamlı bir artış gösterdi. Gelişme kısıtlılığı olan fetusların venöz kord plazma örneklerinde trigliserid ve VLDL-kolesterol düzeylerinin yanı sıra HDL-kol ve serbest yağ asiti konsantrasyonlarında da anlamlı bir artış gözlenirken, arteryel kord plazma düzeyleri; trigliserid, total kolesterol, LDL ve VLDL-kol. düzeylerinde artış gösterdi. Maternal ve umbilikal kord kanı düzeyleri karşılaştırıldığında ise; kontrol grubunda çalışılan tüm parametreler venöz kord kanında anne kanına göre anlamlı ölçüde daha düşük bulundu. Arteriyal kord kanında ise venöz kord kanı ile karşılaştırıldığında; yalnızca serbest yağ asiti konsantrasyonları istatistiksel olarak anlamlı artış gösterdi. Benzer şekilde, IUGR grubunda venöz kord kanı anne kanına göre tüm parametrelerde azalma gösterirken , arteriyel plazmada, kontrol grubuna ters olarak serbest yağ asiti düzeyleri venöz kord plazma örneklerinden anlamlı ölçüde düşük bulundu. IUGR olgularında hem anne hem de fetusta hipoglisemik ve hiperlipidemik bir durum gözlenirken, her iki grupta da fetal dolaşımdaki glukoz ve lipid düzeylerinin annenin metabolik durumundan bağımsız olarak değiştiği saptandı. Ayrıca, gelişme kısıtlılığı olan olgularda fetal yağ asiti kullanımının olası artışına bağlı olarak , arterial kord yağ asiti konsantrasyonlarında belirgin azalma görüldü.

Abstract

OBJECTIVE: The aim of this study is to evaluate the changes in measures of carbohydrate and lipid metabolism in maternal and fetal circulation of pregnancies with normal and growth restricted fetuses. STUDY DESIGN: This study included 36 women whose gestational ages were between 33- 41 weeks. The subjects were divided into two groups; 23 women who had appropriate growth fetuses and 13 women who were diagnosed as having growth restricted fetuses. Maternal venous blood specimens were obtained after the onset of labor and also just after the delivery, umbilical cord venous and arterial blood specimens were collected. Glucose, total cholesterol, triglycerides, HDL, LDL, VLDL-chol and free fatty acid (FFA) were assayed in the plasma of the samples. RESULTS: Maternal venous blood specimens in the IUGR group showed a significant decrease of glucose concentrations when compared to the control subjects, whereas total chol triglycerides, LDL and VLDL-chol. levels showed a significant increase. Venous cord blood specimens of the growth restricted fetuses similarly showed increased triglycerides and VLDL-chol. concentrations and also increased levels of HDL-chol and FFA. The differences between two groups with respect to arterial cord plasma levels of lipids were similar to those observed in maternal blood. All the variables were found to be decreased venous cord blood when compared to the maternal blood both in the normal group and in the growth restricted ones; while comparison of arterial cord blood with venous cord blood showed a significant increase only in FFA levels in the control group but contrarily a significant decrease in the IUGR group. Correlations between maternal blood and venous cord blood were not found to be significant for any variable in both groups. CONCLUSION: Maternal and fetal metabolic status appear to be hypoglycemic and hyperlipidemic in IUGR cases. Fetal glucose and lipid levels seem to change independently from the maternal metabolic status both in normal pregnancies and in the IUGR group. Also; in the growth restricted fetuses fatty acid utilization in fetal circulation seems to increase causing significant lower levels in the arterial cord blood.
