Can repeated human chrorionic gonadotrophin treat empty follicle syndrome?

Özet
Boş follikül sendromunun kesin etyolojinin bilinmemesine rağmen, özellikleri ile tutarlı olarak ovulasyonu tetikleyen hCG dozunun tekrar uygulanması sonrası başarılı şekilde ikincil oosit toplanması olasılığı çoğu vakada yayınlanmıştır. Insan kaynaklı hataları bir tarafa bırakırsak boş follikül sendromu şimdilerde etyolojisi herne olursa olsun hCG kullanılabilirliğindeki azalma çoğu vakada temel sebeb olarak görülmekte ve kabul görmektedir. Bu raporda ikinci hCG dozu ile başarılı bir şekilde oosit aspire edilen fertilizasyonun gerçekleştiği ancak bu fertilize oositlerde yarıklanmanın gerçekleşmediği vaka sunulmuştur. olgumuzda Ilk aspirasyon günü -hCG düzeyi serumda 10,7 mIU/mL ve folliküler sıvıda 1mIU/mLnin altında bulundu. IU hCG verilmesini takiben 24 saat sonra yapılan aspirasyon zamanında ise -hCG düzeyi serumda 292 mIU/mL ve folliküler sıvıda 52,5 mIU/mL bulundu. Bizim sonucumuz boş folikül sendromunun klinik disfonksiyondan çok ilaçla ilgili bir problem olduğu teorisi ile uyumludur .
Abstract
Despite the uncertain aetiology a consistent feature of empty follicle syndrome is the posibility of succesfully retrieving oocytes following a repeat ovulation triggering dose of HCG and a second oocyte recovery procedure in the majority of reported cases. Apart from human errors in the aetiology of empty follicle syndrome, it is now well accepted that decreased HCG availability,whatever its origin, seems to be a fundamental cause in many of the published cases of empty follicle syndrome. In this report we presented a case succesfully aspirated oocytes with a second dose of hCG. Fertilization of these oocytes was succesfull but this fertilization oocytes did not progress to cleavage in our patient. In our case first retrieval day serum -hCG level was 10,7 mIU/mL, follicular fluid -hCG level was under the 1 mIU/mL. Second retrieval day (24 hours after second hCG dose) serum -hCG level was 292 mIU/mL, follicular fluid -hCG level was 52,5 mIU/mL. This case supports that empty follicle syndrome is a drug related problem rather than a clinical disfunction.
